Job Title: Wellbeing Practitioner

Hourly rate: £14.54

Hours: 21.5 hours per week (working Tuesdays, Fridays & Saturdays)

Reports to: Manager

Accountable to: Manager

Job Purpose:

  • The Wellbeing Practitioner will be part of the integrated team based at a Bluesci Wellbeing Centre. to support the delivery of the service in line with the Living Well and neighbourhood model across Trafford.
  • The Wellbeing Practitioner will support the role of Wellbeing Volunteers who will be an integral part of the service,
  • The Wellbeing Practitioner will work collaboratively with individuals with mental health issues / long term conditions / medically unexplained symptoms to identify their health, wellbeing and/or education/employment goals and aspirations.
  • Behaviour change is a key element of support offered by the Wellbeing Practitioner and this will be achieved by helping build individuals motivation, confidence for change, connecting with community assets and help reduce social isolation, health management (including attending health screening appointments, other health appointments, increasing physical activity & improving mental health & wellbeing in the process.

Core Duties and Responsibilities

The Wellbeing Practitioner will:

  • Have day-to-day responsibility for a defined caseload, and for the setting, monitoring and maintaining of planned & achieved goals over an agreed time period
  • Work collaboratively with individuals to determine the type/nature of support required to access and utilise community resources.
  • Work with Individuals and the Wellbeing Practitioner to identify areas of need and opportunities as indicated by the assessment and intervention.
  • Engage with individuals through the development of a person-centred action plan to achieve the agreed goals, offering face to face support. Linking individuals into a range of services including social prescribing, volunteering and pathways to employment.
  • Support individuals to identify and problem solve practical issues promoting self-care and resilience. Support individuals to attend Voluntary / Community-based groups and opportunities, and/or appointments including; health screening.
  • Work with partner agencies to support identified individuals to volunteer or access education, training, pathways to employment.
  • To ensure that agreed assessment tools (ONS) are used and completed at initial and final appointments, interventions are documented and all records are kept up to date and carried out to a high standard .
  • Be actively involved in contributing to the team’s key performance indicators, participating in evaluation, in the development and implementation of quality initiatives. Assist in improving access to services to individuals from Black & Minority Ethnic (B&ME) groups.
  • Ensure effective verbal and written communication with all staff, Volunteers, individuals using the service and their carers in order to maintain a high-quality Service

General Duties and Responsibilities:

  • To support the team to work collaboratively and sensitively with individuals by role modeling and example. And where necessary manage the complaints process.
  • To be responsible for the allocation of work and implementation of Service delivery in accordance with regulatory and operational standards, policy, and procedures for the service
  • To review the service, and make recommendations for service improvement, policy development and operational standards.
  • To provide supervision, appraisal, personal development and career progression of individual team members in accordance with Bluesci policies and procedures.
  • To engage in regular supervision and appraisal with the Manager
  • To be responsible for the team of Volunteers & partners compliance with Legislation, National Regulations, Standards and guidance and Bluesci policies and procedures and those of GMMH.
  • To coordinate and monitor a range of delegated budgets assigned to the post (Volunteer expenses).
  • To coordinate the day-to-day planning and induction of volunteers
  • Maintain accurate and up to date records safely and confidentially in accordance with Bluesci policies and procedures
  • To be responsible for the collection and analysis of performance management information and to manage this information effectively to achieve the service objectives, and targets for improvement.
  • To contribute to the Service plans and be responsible for effectively communicating strategic planning and decisions to the team and key partner.
  • To be responsible for identifying, assessing and managing risk to individuals, employees, the organisation and resources arising from the work delivered by the service.
  • Develop effective partnership working with colleagues in related teams/agencies both in terms of providing effective responses to individuals accessing the service and the development of integrated services to fulfil the Social Inclusion and Recovery and Social prescribing Agenda.
  • To carry out all duties in accordance with Bluesci’s Equal Opportunities Policy and other policies designed to protect employees and users of the service from harassment. Bluesci’s Directors are required to check with the DBS the possible criminal background of staff and volunteers who apply for work with vulnerable people.

KNOWLEDGE

Willingness to learn the values and principles underpinning person centered, recovery, social prescribing and positive therapeutic relationships. E

Willingness to learn & gain knowledge and understanding of best practice and national developments relating to Recovery, Social prescribing & Social Inclusion. E

Willingness to learn & gain Knowledge of the management of people within the field of Recovery, Social Prescribing & Social Inclusion. E

Knowledge of budget/resource management. E

Knowledge of risk management processes [Positive risk taking and self-management] and the ability to assess and manage risk professionally and organisationally [risk to staff/resources] E

Knowledge of systems management and ability to monitor the effectiveness of those systems. E

QUALIFICATIONS

EXPERIENCE

At least 1 year experience of working with people or in a wellbeing role. E

Prior experience of coaching, supporting, motivating individuals. E

Leadership Qualification PWP / Counseling or equivalent qualification. D

Prior experience of psychologically informed interventions and guided self help / mentoring/co-working/supervision. D
