Job Title: Support Worker
Salary: £12.45 per hour (rising to £13. 70 from 1st April 2023)
Hours: 37.5 per week
Location: Manchester
Contract: Until 30th June 2023 with potential to extend for a further 2 years
Responsible to: POPS’ Operations Manager

PURPOSE OF THE POST
The Welfare Services Programme is a needs-led, person-centred service, offering a range of 1-1 support and interventions to improve and support the wellbeing of men and women who are subject to Probation supervision, many of whom will have multiple or complex needs.
The post holder will be based at POPS’ Welfare hub in Manchester and will operate on a case work basis, to carryout initial assessment, provide 1-1 support and deliver a range of small group sessions to address and resolve identified welfare/wellbeing issues.
The role will peripatetic, requiring travel to various community and Probation locations across Manchester
TASKS
- Manage case load of mandated referrals from Probation Service
- Carry out needs assessments with all individuals referred to the Welfare Programme Outcome Star Assessment Tool.

- Produce and agree an individual action plans within the timescales set by The Probation Service national standards
- Deliver advice, support and interventions to people on Probation.
- Make supported referrals for individuals to access to specialist provision where required, i.e. Debt management services and track progress
- Provide timely and accurate information to Probation Practitioners
- Provide culturally appropriate support for Men and Women from diverse backgrounds
- To work with each person on Probation to achieve agreed goals, milestones and outcomes by providing a diverse and bespoke range of 1-1 support and small group work interventions
- Co-ordinate service user referrals to partner agencies for specialist interventions, support, or activities (i.e Mental Health Services .
- Maintain up to date case records using Probation service and POPS’ In House electronic CRM systems.
- Work on flexible basis, some travel across locations in Manchester will be required
- Undertake any other duties as requested by POPS’ line manager.
