An exciting opportunity has arisen to join the newly established Virtual Wards and be part of a friendly, supportive multi-disciplinary team.
This post will be part of a seven-day service based at Chesterfield Royal Hospital but may include some off site/community working. The post includes joint working across all the Virtual wards (Cardiology frailty and Respiratory) This post forms part of the Virtual Ward implementation across Derbyshire and is supported by NHS England and the local ICB.
The role involves monitoring patients and providing treatment and care remotely, utilising state of the art technology via a Virtual Ward. Within the role, you will carry out the assessment, planning and implementation of nursing care and evaluation of virtual ward interventions to meet the needs of the patients.
