The function of the social work role is to work with transitioning young people from Children Services to the Adults Learning Disabilities Team, working with young adults, their families and carers who are ordinary residents within the borough of Hammersmith and Fulham. This includes service users outside of the borough in complex residential and educational placements and those in supported living accommodation.

The post holder will possess developed specialist knowledge of social work practice in young people and adults with a learning disability and be registered with Social Work England. The role consists of working closely with other organisations to ensure the safeguarding of our customers, and to assess and intervene and using professional judgement employ a range of interventions promoting choice, control and independence.

The post holder will play a key role in ensuring young people who may be eligible for Adult Social Care under the Care Act have an effective transition from Children’s services. The role will include casework and life planning with young adults, their families and professionals. The post holder’s practice will be responsive, inclusive and community based. They will work within the legislative frameworks (Children and Adults) and agreed Local Authority policies and procedures. This will include taking appropriate safeguarding action as and when required. They will ensure that as part of the assessment in accordance with s.9 and s13 or the Care Act 2014, NHS Continuing Healthcare is considered for all individuals, and where it is considered an individual has health care needs, a referral to the relevant health authority is made in a timely way.

The post holder will act in accordance with the principles of personalisation, ensuring that care and support are person centred and as far as possible put the people with whom they work in control of their lives. Tasks will include carrying out assessments of need, planning of services and reviewing outcomes with the individual, their personal networks and support providers.

As a social work professional, the post holder will bring significant expertise to complex cases in our specialist Transition to Adulthood team. The team is based within the Adult Learning Disability service and works closely with the Disabled Children’s Team and SEN Preparation for Adulthood keyworkers.

Role Responsibilities:

The post holder will:

  • Demonstrate a “can do “attitude and an ability to work autonomously and be self-directed albeit under management supervision and within the limits of the social work role.
  • Manage an allocated workload of young people, with a range of needs, transitioning from Children’s to Adult services and ensure that there is effective communication between the family and all key parties to ensure a smooth transition into adulthood.
  • Undertake eligibility and outcome-based care assessments in line with the Care Act 2014; identify creative solutions which promote enhanced/ ongoing independence and which minimise the dependence on the Local Authority and EHCPs.
  • Undertake Mental Capacity Assessments and Best Interests Decisions
  • Liaise and negotiate with other professionals including CCG, Education, Police, council departments, statutory and voluntary agencies to ensure that the best possible service is provided for young people.

Role Specific Qualifications:

You must be a qualified Social Worker with Social Work England registration to be eligible for this role.
