The North Community Nursing and Therapy teams are looking to recruit two new Band 3 Therapy Healthcare Assistants working across all Teams in the North Locality for a 1 year fixed term contract.
One successful candidate will have the opportunity to work in NN1 and NN3 teams covering Fakenham, Wells, Holt, Sheringham and Aylsham GP practice patients and the other successful candidate will have the opportunity to work in NN2 and NN4 teams covering Cromer, North Walsham, Wroxham and Brundall GP practice patients.
These new and exciting roles will work alongside the Occupational Therapists & Physiotherapists to assist in the delivery of high standards of health care to patients within their own homes, care homes, the travelling community, community hospitals and in clinics, by following prescribed treatment & rehabilitation programmes. they will also work within a reablement philosophy, using strengths based assessments and actively promoting independence for the patients.

- To work within the Therapy team to provide rehabilitation and treatment programmes to keep patients safe in their own homes.
- To contribute to the delivery of care packages for patients with long term conditions, palliative care and rehabilitation needs for example, to achieve quality of life and independence where possible.
- To work within the integrated team to facilitate early discharge from hospital
- To work within the integrated team to prevent unnecessary admission to hospital
- To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.

- Engage with referral to discharge standard processes
- To be aware of the demand and capacity model which will reflect workload needs at any given time
- Work in a flexible manner
