Therapy Service - Stroke

We have an exciting opportunity for a Band 4 Therapy Assistant Practitioner to join our established Stroke Service at Southport & Ormskirk Hospitals.
We are currently in the process of setting up an Early Supported Discharge (ESD) service to compliment the inpatient stroke service. This service will operate across the North Sefton and West Lancashire localities and will facilitate discharges from the hospital and provide rehabilitation within the community.
You will be part of a friendly and experienced multi-disciplinary team, working across Physiotherapy, Occupational Therapy, Speech and Language Therapy and Dietetics to provide excellent patient-centred care. There will be opportunities to work both within the hospital setting and within the community.
You will be encouraged, and required, to further develop your skills and knowledge related to our client group to enable you to provide high quality holistic care. This position would suit a candidate in possession of an assistant practitioner foundation degree or equivalent qualification.The position will be supported through supervision from one of our senior clinicians. The post holder should have experience in acute stroke rehabilitation and understand community delivery.
The post holder will be expected to work flexibly and to support weekend working and may include participation in weekend/Bank Holiday rotas.

JOB SUMMARY
1. To assist the qualified Therapists in their administrative duties for the functioning
of the Therapy Services.
2. To take responsibility and manage own delegated clinical caseload.
3. To provide assistance for the Therapy and Rehabilitation Services in delivering
the required range of therapeutic interventions and support to the client group.
4. To undertake assessment, treatment and evaluation of treatment and intervention
as required or as directed by HCPC registered Therapist.
5. To participate in audit and service development.
6. To accurately record, obtain, collate, and maintain patient records both manually and electronically.
