We are pleased to announce we are recruiting new Band 4 Therapy Assistant Practitioner posts in our Interim community team at University Hospitals Dorset. Our team aims to provide early supported discharge for elderly patients from Bournemouth and Poole acute hospitals as a part of our hospital discharge to assess pathway.
The Interim Care Team at the Royal Bournemouth Hospital are an extremely dynamic, innovative and supportive multidisciplinary team looking to improve and integrate health and social care for older people in Dorset and its surrounding areas. We have been successful in facilitating patients to return to their own homes and therefore reduce their hospital stay which has been demonstrated to significantly impact health and wellbeing outcomes.
The key clinical responsibilities of the position will be to provide community assessments and review and update the care and goal plans of our patients in their own homes after discharge from hospital with the support of the clinicians within the team. Our work in the community involves supporting patients with personal care and medications therefore we are looking for staff with hands-on experience of health and social care. The successful post holder will also support the delivery of education and training to our therapy assistants within the team around the core skills for community care and rehabilitation.
Base Location: Royal Bournemouth or Christchurch Hospital

This is an exciting new role suitable for experienced therapy assistants with level 4 or above qualifications to continue your career development within our community assessment service. The role will include 7 day working over part time hours. Our team currently has bases at both Bournemouth and Christchurch hospital and the role will involve community work and for this reason a car owner/ driver is essential. We support patients across South Dorset and West Hampshire and staff will be paid for time and mileage incurred during work as per national terms and conditions.
