West Dunbartonshire HSCP is seeking an AHP / RN Team Lead who is passionate about making a visible and rewarding difference to people by helping them live independently in their own homes.
As the Team Lead you will lead an integrated team and deliver a high quality service in compliance with statutory responsibilities, Local and National guidelines and the policies and procedures of the organisation.
The Team Leaders within Integrated Adult Services have a variety of professional backgrounds and the ethos of support, collaboration and high quality care is central to the Service. A comprehensive induction will be provided, along with an ongoing programme of support and supervision.
This post has a focus on adults with physical disabilities, and includes a role within our HSCP GP led Youth Enablement Service: a specialist clinic-based rehab service for adults aged 16-65 with a neuro diagnosis.
You will provide clinical oversight and caseload supervision / management to the multi-disciplinary team which comprises Occupational Therapist, Physiotherapists, Social Worker and Social care staff, Rehab Support Workers.
You will work closely with the Integrated Operations Manager to support and encourage the continuous improvement of service delivery and performance.
This integrated and co-ordinated approach will deliver person centred care to support people being discharged from hospital and also reduce the risk of hospital admissions and provide rehabilitation in peoples own homes.
