Job description
We’re seeking a dedicated and enthusiastic support worker who wants to help make a difference to the lives of vulnerable adults in our local community.
You’ll join a friendly and professional team, working with colleagues and key partners to support people to live an inspiring life and help them to achieve their goals.
Role Responsibilities- Carrying out home visits and helping people to achieve tasks that have been set out in their individual support plan
- Managing a case load with support from the persons allocated worker
- Partnership working with colleagues in adult social care and with other key professionals/agencies
- Ensuring all work and interactions are recorded accurately and in a timely manner
- Excellent communication and listening skills
- Practical and organised
- Good computer skills and is confident with using IT systems
- Able to work independently, using your own initiative
- Access to a vehicle to use for work
