Job description
We’re seeking a committed and enthusiastic Support Worker to join our Hospital Discharge and Community team. This is a rewarding role where you’ll make a difference to vulnerable adults and achieve positive outcomes.
You’ll assist people to return home and provide practical assistance to support safe discharges, this includes supporting with tasks such as shopping, arranging cleaning, protection of property and any other tasks that will ensure a safe discharge from hospital or other discharge pathways in the Community.
You’ll follow a support care plan created by a social worker or social work assistant to meet the service user’s outcomes.
We’re a supportive team and are committed to the development of our colleagues. This is a fast-paced environment where you’ll experience working with a variety of health and social care professionals.

Role Responsibilities- Carrying out home visits and helping people achieve tasks that have been set out in their individual support plan.
- Managing a case load with support from the persons allocated worker.
- Partnership working with colleagues in adult social care and with other key professionals/agencies.
- Ensuring all work and interactions are recorded accurately and in a timely manner.
- Excellent communication and listening skills.
- You'll be practical and organised.
- Computer skills and confident using IT systems.
- Able to work independently, using your own initiative.
- Access to a vehicle to use for work.
