SUPPORT WORKER JOB DESCRIPTION

Job Title Support Worker

Job Purpose To implement care plans by directly providing practical, physical, social and emotional care in client’s own homes. To create a supportive atmosphere in which clients can achieve maximum independence. To follow My Homecare’s policies and procedures.

As a support worker you must be sensitive and responsive to the race, culture, religion, age, disability, gender and sexuality of the client who is receiving care and their representatives. The support worker will: -

  • Look after the physical, emotional, cultural and social needs of the client, using a person centred approach.
  • Observe and promote the client’s choice, independence, dignity, privacy, fulfilment and other rights.
  • Create and maintain good professional relationships with clients, their family and friends and other stakeholders
  • Actively support other support workers
  • Adhere to all regulatory and statutory obligations and My Homecare’s policies, procedures and guidelines
  • Actively market My Homecare and promote a positive, personal and professional profile, ensuring the good reputation of My Homecare at all times

INTRODUCTION

Our domiciliary care services provide care and support to individuals or families in their own home. The service is required due to infirmity, illness, incapacity or social problems that prevent our clients from undertaking their various self- care duties. Our aim is to promote and prolong our client’s independence in their own homes.

Predominantly, our client base is elderly, though illness affects all ages. Therefore, the support worker should be able to care for clients from the age of 18 years old.

Our support worker services provide personal care and support, and we also employ support staff who are responsible for assisting with domestic duties in the client’s own home.

Care work is a career. Our support workers receive practical training to complete their duties, as well as studying for Certificates and Diplomas in Care, so as to promote their career in care.

MINIMUM REQUIREMENTS

Basic Academic Qualifications preferred

Previous experience in care is preferable, but not essential, as full training will be given

SVQ level 2 in Health and Social Care OR willingness to work towards this qualification

JOB HOURS

Flexible, depending on the needs of the business

Part time and full time hours available

Some evening and weekend working is required

Sleepovers, waking night-shift and Live in Care positions may be available on request

Main Activities The specific requirements of each assignment will vary and these will be discussed with
you before you accept the assignment.

You will provide care according to the client’s specific diagnosis and needs. Your duties

will include:

Care Provision

  • To provide personal care and support to clients with a wide range of needs,

illnesses and disabilities

  • To know and understand the care and support of the client
  • To undertake the tasks detailed in the client’s Personal Support Plan using a

person centred approach and in the least intrusive way

  • To encourage the independence and motivation of the client and not foster dependent behaviour
  • To provide input into the care and support plans of clients by regularly feeding

back to the Care Co-Ordinator or Manager

  • To assist the client to get up in the morning and to go to bed at night
  • To assist clients to wash, bath and shower, including the use of bath aids where

applicable

  • To assist clients to dress and undress
  • To assist clients to look after their skin, teeth, hair and nails, including assistance

to shave, if applicable

Job Responsibilities To assist clients with toileting. Continence management and personal hygiene,
including catheter care and Urosheath care

  • To assist clients with stoma care, including emptying colostomy bags
  • To assist clients with their medication at the agreed level of support, as detailed

in the client’s Personal Support Plan

Management.

  • To use manual handling equipment safely and correctly
  • To take responsibility for the safe handling of property and equipment belonging to the client
  • To maintain good communication and effective working relationships with clients
  • To provide companionship to the client, actively talking and listening to them about their interests
  • To help the client maintain contact with their family and friends
  • To accompany the client on trips into the community
  • To assist the client to manage their personal affairs
  • To ensure as safe as possible the living environment for the client, whilst respecting the client’s choice and rights

Recording and reporting

  • To maintain detailed accurate records in respect of care and medication support given and tasks undertaken
  • To regularly read care and support plans, acknowledging changes
  • To protect the confidentiality of all information relating to the client and not divulge information to anyone who is not authorised to receive it.
  • To promptly report to the office or Out of Hours Care Co-ordinator any issues concerning the care, support, well- being or behaviour of the client and update records accordingly
  • To continue to monitor where concerns have been reported and recorded
  • To recognise the signs of abuse and immediately report abuse or suspected abuse to the Manager
  • To report any complaints to the office or Out of Hours Care Co-Ordinator
  • To contact the office or Out of Hours Care Co-Ordinator if running late

General

  • To dress appropriately, wearing the uniform and using the personal protective equipment provided by My Homecare
  • To seek out best practice and look at innovative ways to improve the quality and efficiency of service delivery
  • To attend and participate in regular My Homecare meetings and any other relevant meeting
  • To attend in house and external training pertinent to the role of support worker
  • To observe all Health and Safety rules and take reasonable care to promote health and safety of self and others and raise any concerns to the Manager
  • To aim to ensure everyone has equal access to services and employment
  • Any other duties requested by Senior Management, which are within the scope of the post, e.g. shopping, ensuring that all receipts are kept.
  • To work within the 24- hour rota system, which includes Bank Holidays, weekends and evenings. My Homecare will attempt to provide working times suitable for employees, though this is not always possible
  • To be contactable via mobile phone if necessary information needs to be communicated in respect of a client

ESSENTIAL REQUIREMENT – AN ENHANCED PVG DISCLOSUE CHECK

Conditions

  • My Homecare will require an Enhanced PVG Disclosure prior to commencement of work, which the company will apply for
  • All initial practical training will be paid at the usual rate, unless in cases where the employee fails to commence employment at the agreed rate.
  • Offers of employment are subject to the Company receiving two written references, one of which being from your last employer
  • Employees are entitled to 28 days’ paid annual leave.
  • All posts are subject to a six month’s probationary period
