Due to staff reorganisation, we are able to offer an exciting opportunity to work both within the therapy led day service and our Connections Hub. We are looking for a diverse and experienced Support Worker to take on the challenges presented by our drop-in service. However, we also need a support worker to join our therapy led team at the centre to help deliver groups and 1:1 sessions improving the lives and independent living skills of individuals with brain injury. We are offering this post as a joint post covering both the centre and connections (Full time ) or as an opportunity to work in one of our centres. JOB DESCRIPTION - SUPPORT WORKER AT EITHER OR BOTH SECOND CHANCE HEADWAY / CONNECTIONS HUB

Responsible to: Clinical Manager

Location: Second Chance Headway Centre, Wakefield

Salary: £10.50/hr - up to £12.50/hr

Pay will reflect level of job responsibility and previous experience.

Hours: 21 – 35 hours (over 3 – 5 days)

Normal working hours are 9-4pm Monday – Friday (Please note : Connections opens Tuesday, Wednesday and Thursday. )

PURPOSE

  • To support service users appropriate to their individual needs, ensuring that each service user leads a full and valued life and receives quality services.
  • To participate in the development of service users’ individual and group programmes and complete written records as necessary in accordance with guidelines provided. Contribute to creating an environment that aids social interaction for service users.
  • To work as an integral part of the team, sharing the charities ethos and in line with the mission statement. Maintain a professional, supportive and approachable manner at all times. Ensure the charities reputation remains intact.
  • The potential opportunity to support in our Connections hub providing support, advice and signposting to individuals and their families impacted by brain injury. Engage directly in assessment, good record keeping, practical support and community visits.

MAIN CENTRE RESPONSIBILITIES

  • To assist in the development of service users’ individual and group programmes, helping them to relearn old skills which have been lost as result of a brain injury, or acquire new skills to help minimise the impact of the injury.
  • Develop, plan and deliver a wide variety of groups which have been identified as beneficial to a small group of members (Max 10 people) with specific needs.
  • Deliver 1:1 sessions, adhering to specific guidance and agreed goals following assessment by a member of the multidisciplinary team.
  • Deliver holistic support ensuring relationships are developed with the service user and those directly involved in their care
  • Attend keyworker meetings, complete risk assessments and ensure all documentation is completed and kept up to date
  • To develop a knowledge and understanding of service users’ needs and aspirations to enable them to effectively communicate their choices and intentions. Similarly enabling service users to understand and communicate with others
  • Enable service users to identify and work towards individual goals
  • Work alongside the MDT to identify realistic goals and help breakdown safe ways of working towards these goals
  • Assist service users to express themselves in the centre, home and community settings where appropriate
  • Assist service users to understand the needs of others where appropriate
  • Accompany service users on activities and actively participate as required and directed by the line manager, ensuring that service users get to and from activities safely and promptly
  • Assist in completing and updating risk assessments
  • Observe and report any concerns to your line manager
  • Ensure you maintain professional boundaries and safeguard service users at all times
  • To act as key worker and advocate for certain designated service users, liaising closely with other members of staff.
  • Record keyworker updates in written or online files
  • Attend keyworker meetings
  • Report concerns immediately to your line manager
  • Ensure you are up to date with all policies and procedures

MAIN CONNECTIONS HUB RESPONSIBILITIES

An opportunity has arisen to work in our Connections Hub as well as at the main centre. This is a diverse and challenging role working with individuals with brain and their families, running support groups, liaising with statutory, health and third sector services and developing your ability to work autonomously. Support and training will be provided by our MDT.

  • To play an active role in the delivery of support , advice and signposting in our drop in service.
  • Maintain a welcoming and safe environment.
  • Assist in the assessment and recording of these details.
  • Support service users in the community where appropriate
  • Ensure you adhere to GDPR guidelines and policies
  • Respect the privacy and dignity of the individuals you support.
  • Assist the clinical team with their support plans – this may include but is not limited to:

Helping with benefit paperwork, bus passes, brain injury identity cards.

Filling out forms, making phone calls and attaining details needed.

Making GP appointments

Supporting service users to make phone calls, understand paperwork etc.

Liaison with other third sector organisation involved in an individual’s care.

  • Coordinate and support volunteers at the Connections Hub

OTHER DUTIES

  • Work as a member of the team, contributing to the team ethos and supporting service users and colleagues appropriately
  • To maintain a safe environment at the rehabilitation centre.
  • To complete written and online records as necessary.
  • To attend, participate and contribute to, staff meetings, reviews, person focused meetings, training and supervision sessions as requested. To complete an annual online programme of training.
  • Identify specific or desired training needs in supervision sessions / appraisal to aid personal development
  • To be aware of the need for confidentiality, ensuring that information regarding service users is only discussed with the appropriate people.
  • Foster good relationships with volunteers.
  • Adhere to the Covid 19 policy and complete any tasks / actions required. This may include but is not limited to;
  • Testing
  • Cleaning protocols
  • Social distancing
  • Use of face masks/PPE as required

GENERAL

  • Actively participate, where appropriate, in conferences, seminars and external meetings in order to promote the work of the charity and the interests of adults with acquired brain injury.
  • Be aware of all policies and procedures in place at the charity
  • During the course of your work the job holder will conduct themselves in a professional manner at all times, acting as a role model to service users, colleagues, students and volunteers, contributing to the delivery of quality services
  • Undertake any reasonable duties for the charity as required by your supervisor.
  • The job holder will be subject to a Criminal Records Bureau Disclosure check.

QUALIFICATIONS

  • Desired - previous experience of working with people with brain injuries or hidden disabilities, NVQ 1- 2 care/rehabilitation or equivalent – but other skills, qualifications, experiences etc. will be considered.
