We are looking for compassionate and caring support workers who have the skills to care for our residents with respect, dignity, empathy and warmth.

The successful applicant will be required to undertake all aspects of nursing support including activities with residents and work a variety of shifts; nights, days and weekends. In return we offer the right people the opportunity to develop their skills and knowledge in Dementia care.

1. Communication and relationship skills

Able to communicate, establish and maintain relationships with a wide range of internal/external people including staff within Darland House, relatives and visitors

Able to provide, receive and record routine information as appropriate.

Able to use communication & listening skills to overcome barriers to understanding.

Able to motivate, negotiate, reassure and empathise with individuals
1. Knowledge, training and experience

Has an awareness and understanding of personal care and related procedures for meeting individual’s special needs

Able to carry out a range of routine work procedures, knowing how to prioritise work and recognise when to call for assistance and/or report changes in the resident to the qualified member of staff.

Willingness to take part in further specialist training to extend skills and support the care provided to the residents. Able to share this with other members of the team.

Able to work as part of a multi-professional team and offer support as appropriate. Have an understanding of the different roles
1. Analytical and judgement skills

Ability to follow care plans and risk assessments, monitoring standards of care and judging situations as required. Alerting qualified staff to any issues regarding patient condition immediately

Able to prioritise duties, showing clear judgement and offering solutions as appropriate
1. Planning and organisational skills

Works as a supportive member of the team, assisting others to ensure delivery of high quality, person centre service

Able to plan workload and alert qualified staff immediately of any problems or conflicts
1. Physical skills

Physically able to coordinate practical activities with residents e.g. manoeuvring wheelchairs, using hoists

Able to show understanding of health & safety issues concerning the manual handling of residents as indicated in plans of care and risk assessments

Able to show understanding of own responsibilities for health & safety including ability to take appropriate action if required.
1. Responsibility for patient / client care

Provision of personal care to residents as indicated in plan of care and risk assessments, ensuring safety at all times.

Encourage patient independence as appropriate. Residents should have as much control as possible over their own health

Monitor standards of care in collaboration with qualified staff

Actively ensure a clean and safe environment in collaboration with other colleagues within the home
1. Responsibility for policy and service development implementation

Follows Medway Community Healthcare policies and procedures, alerting the appropriate person to any difficulties in doing so.

Participation in discussion regarding policies and procedures and any proposed changes.
1. Responsibilities for financial and physical resources

Assist the Head of Service in maintaining budgets within spending limits, making best use of resources available

Maintain duty of care in relation to equipment and resources. Reporting problems/discrepancies quickly and to the appropriate person

Maintain duty of care when handling resident equipment and belongings.
1. Responsibilities for human resources (HR)

Demonstrates own duties to new or less experienced employees and agency staff as required

Demonstrates good understanding and application of the equality and diversity policy
1. Responsibilities for information resources

Correctly and appropriately records personally generated information

Contributes to the updating of patients records

Ensure that confidentiality in respect of residents is maintained. Awareness and application of MCH information governance policies and procedures.
