We are looking for dedicated and passionate Care Support Workers to join the CareHomes Stoke Team. CareHomes Stoke is part of the rapid response service, looking after people in their own home following discharge from hospital or community services.

Main duties include:

Ensure Service Users are at the heart of care delivery and their wishes and preferences enhance their wellbeing

To contribute to the efficient running of the service by attending calls on time and providing Person Centred Care

Document care appropriately using mobile device

Complete all necessary training and take ownership on learning new skills
