Job overview

We have an exciting opportunity for a band 5 rotational post through all of Bury’s Intermediate Tier which includes; Rapid Response, Choices for Living Well bed and home base, Virtual Hospital and Rapid Response. There is then a fantastic promotion structure which can lead to Advanced Clinical Practitioner or Management. It is a full multi-disciplinary team, incorporating both health and social care which enhances all skills. Bury is making significant investment to improve the capacity and capability of intermediate care and related services under the leadership of the Bury Local Care Organisation [LCO]. The Bury LCO consists of the seven main health and social care providers in Bury (BARDOC, Bury Council, Bury GP Federation, the Northern Care Alliance, Pennine Care NHS Foundation Trust and Persona) and the Voluntary, Community and Faith Alliance [VCFA].

Main duties of the job


  • Using your clinical experience and skills you will be responsible for the assessment, planning, delivery and evaluation of programmes of care, both for a specific group of patients as the named care provider and in supporting the care of all patients on the ward / unit
  • You will assist in the management and organisation of work, as required, ensuring effective communication is maintained at all times and will provide support for junior staff on the ward (newly qualified staff will receive initial support).
  • You will play a key role in the education, development, mentorship and supervision and assessment of all learners and unregistered
  • You will undertake nursing interventions consistent with evidence based practice, transferring and applying knowledge and skills to meet patients’
  • Be able to work collaboratively with all members of the MDT (Multi-Disciplinary Team) and to make referrals as
  • To provide support and care for the patient and his/her family respecting their need for privacy and
  • To monitor the effectiveness of nursing care and adjust the programmes of care where indicated.

Bury is making significant investment to improve the capacity and capability of intermediate care and related services under the leadership of the Bury Local Care Organisation [LCO].


The Bury LCO consists of the seven main health and social care providers in Bury (BARDOC, Bury Council, Bury GP Federation, the Northern Care Alliance, Pennine Care NHS Foundation Trust and Persona) and the Voluntary, Community and Faith Alliance [VCFA].


The LCO is working with local Commissioners to deliver the integration and transformation of local health and care services as part of Bury’s Locality Plan. This includes plans to enable more people to have their health and social care needs met at home and in the community and avoid unnecessary hospital or care home admission.


The new delivery model for the Intermediate Tier of services is designed to bring together health and social care into a Bury-wide integrated team delivering intermediate care services under single leadership. The key deliverables are to:

  • Introduce home-based care into Bury’s intermediate care offer;
  • Create additional enhanced Intermediate Care [IMC] beds through the dual registration of Killelea House;
  • Develop a multi-disciplinary Rapid Community Response

Staff will be expected to work flexibly across the tier of services which includes:

  • Bury Rapid Community response Service
  • Killelea House (enhanced and residential IMC)
  • Home-based IMC and Reablement
