Job description

Practice within the N.M.C Code of Professional Conduct Participate in the assessment of patient care needs, planning, implementing and evaluating the programme of care in accordance with the agreed policies and procedures of the Royal Wolverhampton Hospitals NHS Trust. To communicate regularly with patients and relatives to maintain understanding of care pathways. Liaise with the multi-disciplinary team regularly to ensure timely delivery of care and foster good working relationships. Act as a positive role model.
