Job SCOPE

The post holder’s main responsibility will be to work within the multidisciplinary practice team consisting of administrative staff, a team of clinical pharmacist/pharmacy technicians, nursing team and GPs where you will be fully supported.

We have 5 GP Partners and 2 salaried GPs with two First Contact Paramedics. We have a dynamic nursing team led by our lead nurse. We have an excellent working relationship with our onsite PCN and our patients have access to link workers, a MSK practitioner, a mental health nurse, and most recently our patients can access an Acute Respiratory Clinic, a successful pilot launched at St Pauls PCN and now being rolled out across the rest of the county. We are a training practice and support medical students and registrars.

We would like to recruit a Specialist Paramedic on a full-time basis to provide a mix of online digital consultations (both video and telephone) as well as face-to-face appointments to our patients. We have a practice list size of 11,300.

As a member of our sessional clinical team, our Paramedics work alongside GPs, clinical pharmacists, and other allied healthcare professionals and provides advice on the best value and most effective treatments and services. This work may include providing triage, referral and/or treatment of patients, utilising relevant evidence-based practice, local guidelines, and practice procedures. In addition, you will provide appropriate care based on patient needs, ensuring patients receive a timely and appropriate response when working operationally.

YOUR PROFILE

Evidence of post-graduate education at Level 7 such as Practitioner course, minor injuries & minor illness or advanced clinical assessment is essential.

Ideally, be a non or independent prescriber or be prepared to undertake the prescribing course

Professional registration with the Health and Care professions Council (HCPC)

Post-registration experience in an appropriate clinical setting (at least two years’ experience).

Previous experience of triage/telephone consultation is essential

In-depth knowledge of pre-hospital care & urgent care systems with the ability to demonstrate credibility. Must be able to demonstrate workplace-based learning in a primary (or possibly urgent care) setting.

Knowledge and understanding of current health and social care agendas

The College of Paramedic Diploma in Primary & urgent care (DipPUC)

Main Responsibilities:
Clinical:
Provide the first point of contact for clinical queries from practice receptionists

Assist with telephone triaging

Undertake urgent/same day clinics

Undertake appropriate home visits alongside the GP visiting team.

Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis

Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly

Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs

Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a management plan

Initiate and interpret tests and investigations as appropriate

Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs, liaise with the pharmacy team for advice & support. Offer advice on over-the-counter medication.

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care

Assess, identify, and refer patients presenting with mental health needs according to practice processes

Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care

Record accurate data in the patient records to an agreed standard

Collaborate with other members of the Primary Health Care Team including doctors, nurses and other AHPs, accepting referrals and referring to them for specialist care

Direct referral to and liaison with consultants and other professional agencies

Ensure a high standard of clinical practice is maintained by ensuring care is based on current best practice, embarking upon further training if required and continuing to update clinical knowledge

Quality Requirements

Recognise and work within your own competence and professional code of conduct as regulated by the HCPC and College of Paramedics

Produce accurate, contemporaneous, and complete records of patient consultation, consistent with legislation, policies, and procedures

Be aware of data protection (GDPR) and confidentiality issues particularly within a GP surgery

Use technology and appropriate software as an aid to management in planning, implementation, and monitoring of care, presenting, and communicating information

Review and process data using accurate SNOMED codes to ensure easy and accurate information retrieval for monitoring and audit processes.

Prioritise, organise, and manage own workload in a manner that maintains and promotes quality

Deliver evidence-based care according to national guidelines such as NICE

Assess effectiveness of care delivery through self and peer review, benchmarking, and formal evaluation

Keep up to date with current evidence-based practice.

In partnership with other clinical teams, collaborate on improving the quality of health care responding to local and national policies and initiatives as appropriate

Support and participate in shared learning across the practice and wider organisation

Assist in the development and implementation of policies and procedures

Understand and apply legal issues that support the identification of vulnerable and abused children and adults and be aware of statutory child/vulnerable patients’ health procedures and local guidance with the support from our Safeguarding Leads

Participate in annual professional development review. Personal development will be encouraged and supported by the practice. It is the individuals’ responsibility to remain up to date with recent developments. Your training requirements will be assessed during your annual appraisal and will be in accordance with practice requirements

To work within the HCPC code of professional practice and Scope of professional conduct, H&S at work Act 1974, Data Protection Act 1984, and Access to Health Records 1990.
