Specialist Mental Health Practitioner for Primary Care Reading, England
Job description
We are looking for qualified and experienced Mental Health Nurses, Occupational Therapists or Social Workers to join us in Mental Health Practitioner roles, Monday to Friday across the Primary Care Network, supporting GPs with on the day Mental Health support for patients.These new roles will provide an opportunity for better joint working across primary care and adult secondary care mental health systems and will make it easier for patients to obtain the help they need.

Tilehurst PCN (Tilehurst Village & Chancellor Surgery) - PT post - 22.5 - 30 hours.
New Reading (Melrose, Russell Street, SR & Shinfield & Long Barn Lane) - FT post - 37.5 hours
