Summary Description:
Baystate Health is seeking a talented, experienced Population Health Solutions Manager.
Reporting directly to the Director of Data Management and Analytics. You will be the liaison between clinical, business, and I&T with responsibility for solution delivery and execution of a newly established, highly visible, and impactful Population Health program. Working independently, you are capable of leading by role or influence, as well as fulfilling or supporting many different roles across the continuum.You will collaborate and coordinate with your partners to implement system changes and upgrades, provide technical support, and develop workflow efficiencies through technology use. Responsible for researching and developing inter-facility and service alignment related to IT technology in order to achieve community wide population based electronic patient management.
Job Requirements:
1) Responsible for the implementation of HealtheIntent related population health projects from inception to completion, including management and coordination of related activities.
Understand landscape of key organizations within Baystate Health and community partners and establish credibility quickly
2) Support development of the population health strategy, including key objectives, measures, monitoring and evaluation plans in partnership with business, clinical, quality teams, and leadership
3) Monitor program progress, collect and review relevant population health data and synthesize to support presentation and communication, including visual displays of data, for relevant stakeholders and senior managers
4) Mediates, negotiates, and influences decision-makers. Develops and follows complex communication, and risk mitigation plans
5) Collaborate with the PHO/Baycare and quality leaders for meeting quality metrics and requirements for value base contracts/Medicaid ACO, etc.Review and monitor compliance with the federal and state regulations.
6) Coordinate with finance, clinical and quality to develop system and program level KPIs.
7) Co-develop and prepare project/program budgets; identify, organize and coordinate necessary internal and external resources
8) Partner with clinical leaders to identify workflow improvements to align with regulatory/payer requirements.
9) Convene and provide overall direction to project/program teams, which may consist of vendors, data staff, associates, and coordinators for the development of project work plans, literature reviews, process improvement, training, data analysis plans, and reports
10) Establishes clear project parameters supporting the success of all internal and client team members. Plans and manages internal and external project communications, ensuring effective, objective, and professional exchange of the project information and deliverables
11) Work closely with product vendor(s) to understand their deliverables and schedule. Manage and foster vendor relationships and ensure delivery meets clinical and business needs
12) Manage the day-to-day operations of Population Health projects, ensuring projects are completed on time, within budget, and to the satisfaction of stakeholders.
13) May act in a project management capacity and be responsible for managing day-to-day logistics and operational needs of the projects/programs, convening and providing overall direction to project/program teams and identifying and addressing problems which may adversely affect project/program work
14) Provide functional supervision of other specific project/program staff as needed
15) Perform other duties as required
Minimally Required Expereince:
Bachelor's Degree: Computer Science, Business Administration, Management, Information Systems or related fields
Minimally Required Experience:
10 years of experience in Solution, Product, and/or Program Management with Project Management experience
Preferred Experience:
8 – 12 years’ experience in managing complex, strategic information technology programs and products in healthcare is preferred and must be recent (within the last five years).
Skills/Competencies:
Broad Healthcare industry experience aligned to Population Health and care provider services
Excels in taking on projects in trouble and bringing them to a successful conclusion; may introduce new methods to manage projects
Evaluates project performance against goals and objectives and takes necessary action to address deviation
A strong understanding of payer quality programs (HEDIS, MSSP, ACO), risk contracts, and value-based care programs
Knowledgeable of structured program management methodologies, applying program management knowledge, skills, tools, concepts, and techniques
Experience in vendor management with knowledge of healthcare software development companies
Ability to serve as a liaison to drive effective, timely solutions that fulfill both strategic and operational needs
Strong investigative, analytical, and interpersonal communication skills, with the ability to work independently or as part of a team
Strong leadership skills, including a history of growing leadership responsibility and career growth in process improvement
Excellent verbal and written communication skills with audiences of all levels
Strong organizational skills and the capability to interpret and handle a vast array of information
Able to balance information systems requirements to support the health system vision within resource constraints, work with Senior Management to prioritize and revise plans; a track record of managing resources judiciously and operating within budget
