We work with the residents of Bracknell Forest to improve their health and wellbeing and maximise their independence. We are a friendly supportive close-knit team.

We’re proud of our community; the care we give and the services we provide. This is why we give our team members the time, support and resources they need to do their job well. You’ll have a manageable caseload, and we will listen to your ideas - if you think something needs to change, we want to know about it. This is your opportunity to take ownership and to change lives for the better.

Do you want to make a positive impact on the lives of the people we support and develop your career within social work? If yes, then Bracknell Forest is the place to work.

Because we’re Bracknell Forest.

This is an exciting opportunity to join the Bracknell Community Mental Health Team for Older Adults. You will be working with health and social care professionals in the team to ensure a high quality of work is delivered.

We are proud to be advocating a good life at home and we aim to help people remain independent as long as it is safe and possible.

We are proud of the care we give, the services we provide and the links and networks we create. But we’re always looking to do more.

We want to help our most vulnerable residents to get their lives firmly on track. It’s why our social workers have the time to get to know everyone they work with. We’re working towards protected caseloads. And we do what we think should be done, not just what needs to be done. Put simply, we change lives for the better.

  • Help to coordinate the resources at hand to identify people with longer term support, to deliver short term interventions utilising strength bases assessment and community assets.
  • To support people with a need for social care to assess their own needs and develop a person-centred plan of support, working together with carers, family and friend networks, health professionals and the voluntary sector.
  • To have responsibility for assessing safeguarding referrals, including carrying out investigations, completing risk assessments, chairing meetings (where necessary) and ensuring that action plans are effective in keeping people safe.
  • To establish good clinical and management practices ensuring high standards of care using community or other available resources.

The benefits you will experience are…

  • Regular opportunities for career development with internal and external training courses.
