Job description

We are looking for a full time Social Prescribing Link Worker to join our multi-disciplinary team within the Macclesfield Primary Care Network.

As a Social Prescriber you will be providing 1:1 personalised support to patients, linking people to sources of support within the community and empowering them to take control over their own health.

You will build trusting relationships with people and work together to create a shared personalised care and support plan, focusing on what really matters to them.

You must be a good listener, have time for people and be committed to supporting local communities to care for each other. You should have experience of working with people facing complex social and emotional challenges, and you will need great interpersonal skills in supporting people from a wide range of backgrounds, community groups and local organisations.

Main duties of the job

As part of the Social Prescribing team you will:

  • Provide individual support to patients to support them to improve their health and wellbeing. This could be done via phone, face-to-face or home visits, as appropriate.
  • Proactively manage and prioritise an individual caseload of patients, supporting them to focus on "what matters to me", and linking them to sources of support within the local community.
  • Take a holistic approach to help people identify the wider issues that are impacting on their health and wellbeing, and together produce a personalised support plan.
  • Encourage people to carry out the goals set out in their plan, and ensure they have the support they need to achieve these goals.
  • Forge strong links with a wide range of local VCSE and community organisations.
  • Promote the Social Prescribing service to patients and staff within the six practices of the Primary Care Network.

Desirable

  • Knowledge of clinical systems (Emis)

  • Understanding of the wider determinants of health, including social, economic and environmental factors and their impact on communities
  • Experience of data collection and providing monitoring information to assess the impact of services
  • Experience working as a community Navigator/ Link Worker/ Social Prescriber
  • Experience in delivering training to user groups / staff teams
