Are you a qualified Social Worker looking for an opportunity to work with a wide range of adults? Swansea Council are recruiting for a number of Social Workers within our Home First Team based mainly in Morriston and Singleton Hospitals, with some work at Gorseinon and Neath Port Talbot Hospitals.

This is a rare and exciting opportunity to join a well-established and supported team offering excellent peer and management support in an inclusive environment.

You will need a recognised social work qualification and to be registered with the Care Council for Wales.

About the role

Hospital social work is extremely wide ranging, our team work with adults aged 18+ who are at their most vulnerable, perhaps frail and unwell or having had a catastrophic event leading to a huge change in abilities or having experienced abuse/trauma or brain injuries. The team works with individuals to support and enable them to be discharged as soon as they have recovered, to live well and as independently as possible.

The team are office based in the hospitals and spend a lot of time on the wards with patients and colleagues. Working closely with the Multi-Disciplinary Team (MDT) that consist of Social Workers, Discharge Liaison nurses, OT's, Physios, Ward Nurses etc and other organisations you will use the 'discharge to recover and assess' model, for an outcome focussed, person centred approach to complex case work.

The Discharge to recover and assess model aims to reduce assessments in the acute setting and to move people onto a reablement or assessment pathway when possible. This also means people should not be making decisions about long term care provision while still in hospital. The team therefore will see some people after discharge at home or in a reablement setting or care home to conclude assessments.

You will be part of a team who often work in challenging and difficult situations yet support each other throughout to obtain positive outcomes for all involved.

All newly qualified (NQ) Social Workers will be required to take part in a First Year in Practice Scheme that is based on guidance by the Social Care Wales. It is also mandatory for newly qualified Social Workers at the end of their First Year in Practice, to also register for and undertake an appropriate qualification in Consolidation of Social Work Practice and to achieve this qualification within the first period of registration as a Social Worker. As a newly qualified Social Worker you will be remunerated on Grade 8, until such time as this requirement is achieved.
