Social Worker
Makepeace Ltd is seeking a Social Worker to join our team in Sutton. We are a rapidly growing organization and are looking for an energetic individual to help with our clients.
Duties and Responsibilities:
- Provide direct case management services to clients in a group home setting.
- Assess client’s needs and provide individualized services based on the client’s strengths and weaknesses, as well as, the needs of the agency, department, and organization.
- Evaluate, plan, implement, monitor, and evaluate the effectiveness of services provided to clients.
- Participate in the development of individualized treatment plans for clients.
- Maintain accurate records of client cases.
- Participate in the evaluation of other agencies.
- Provide consultation regarding issues affecting the community.
- Attend monthly staff meetings and participate in staff meetings as needed.
- Other duties assigned by supervisor.
Education: social work or related field OR Bachelor’s degree in social work with emphasis in human services or related field
Experience: 1-3 years of social work experience required; experience in mental health or homeless prevention preferred; experience working with youth preferred; experience working with individuals with intellectual disabilities preferred; experience working with individuals with developmental disabilities preferred; experience working with individuals living with intellectual disabilities preferred; experience working with individuals living with diverse backgrounds preferred; extensive knowledge of community resources and services available to individuals living with disabilities; extensive knowledge of community resources and services available to individuals who are at risk for homelessness

Salary: £37,625.90-£44,000.00 per year
