Job description
To work as a Care Co-ordinator or Lead Professional (where applicable) with individuals who have serious and enduring mental health difficulties. To develop and provide evidence based programmes of care for service users of working age with mental health problems who are receiving treatment from the Recovery Team, with a focus on social care To ensure a clear focus on recovery and social inclusion. To support and supervise both registered and non-registered staff in delivering care. The role will entail, assessment , support planning and review of social care packages in robust and timely manner, in accordance with The Care Act 2014 requirements.
