- Provide individual assessment for prospective clients and their carers within eligibility criteria and operational guidelines.
- Arrange packages of care for clients and their carers in accordance with assessed need using sensitive and flexible methods and skills.
- Review and reassess individual packages of care according to changing needs and operational guidelines.
- Carry out a full holistic assessment of clients within their own home.
- Ensure that the individuality, personal dignity and confidentiality of clients is respected at all times.
- Create packages of care in which clients will be encouraged to achieve maximum independence, in consultation with other professionals, clients and carers.
- Offer information, advice and emotional support to enable the carers to care effectively.
- Foster appropriate links with relatives, other carers, colleagues and service providers.
- Undertake carer's assessments.
- Identify provision of specialist equipment and minor adaptations as required and make recommendations for appropriate interventions to promote independence and minimise effect of disability.
- Maintain responsibility for a caseload and seek guidance from supervising team leader as appropriate.
- Communicate observations, recommendations and outcomes of assessments to a designated Social Worker / Occupational Therapist and other professionals involved in a case as appropriate.
- Maintain clear and accurate records of casework, manually and/or through the use of information technology, and provide reports / statistical returns.
- Undertake appropriate training as directed by the line manager and co-operate with the introduction of new procedures and / or new technology systems.
- Participate in team meetings, personal development and supervision, contributing to the development of the service by attending working groups and other forums as deemed appropriate.
- Develop and maintain a working knowledge of appropriate resources and care providers, including welfare benefits, social and health care services and eligibility criteria.
- Be conversant with, and adhere to, the Scottish Social Services Council codes of practice.
- Assist Social Workers and Occupational Therapist as required.
- Maintain an awareness of responsibilities as defined by Health and Safety Legislation and ensure an efficient and safe working environment.
