Social prescribing is a role created to assist in empowering individuals to take control of their own health and wellbeing, by focusing on the individual and taking an holistic approach by connecting patients to community groups and statutory services for practical and emotional support. You will support existing groups to be accessible and sustainable and help people to start new community groups, working collaboratively with all local partners. This role will therefore require a significant level of multi-agency working. The role will be mainly dedicated to ensuring ease of access to services for more complex patients throughout the healthcare system. You will be responsible for working with patients, their families and carers, to improve their health and wellbeing by helping them access appropriate services. The role will require management and prioritisation of your own caseload, in accordance with the needs, priorities and any urgent support required by the individual. It is therefore vital that you have a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals/agencies, when the individuals needs are beyond the scope of this role.
