Job description

The primary purpose of the Serious Incident Investigator post is to ensure that the Service Group is fully meeting all its public, statutory and Welsh Government responsibilities in relation to serious incidents. To achieve this the post holder will:

  • lead and be responsible for undertaking robust proportional investigations including root cause analysis investigations in a timely manner as allocated by the Quality & Safety Manager for all Serious Incidents .
  • Comply fully with the Welsh Government Serious Incident/Never Event Reporting requirements – at all stages – ensuring Welsh Government timescales are adhered to.
  • Ensure through effective and timely incident reviews with reports detailing the identified learning are presented to the Service Group, Serious Incident group .
The successful applicant will be involved in working as part of a small team to identify learning for the service following serious incidents. This will include liaising with patients and their families. Working closely with clinicians to identify learning and leading reflective events to support teams to develop learning themes.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

The successful candidate will promote an organisational patient safety culture that focuses on high quality care standards, preventing avoidable harm by using evidence best practice. The Mental Health and Learning Disability service group review the deaths of all patients known to the service to ensure that we understand and learn from each case. The serious incident investigators lead on each review using a Root Cause Analysis structured approach to identify learning from each case.Seriousincident investigators will therefore need to:
  • • understand how patients are harmed.
  • analyse and disseminate patient safety data and other clinical indicators to deliver quality and safety improvements.
  • identify learning by scrutinising practice against policies and procedures.
  • support clinical teams in the prevention of harm.
  • develop the capability of our staff in safety and improvement methodology.
  • work with the Service groups Management Team, interdisciplinary teams, patients, commissioners, stakeholders and voluntary bodies in making safety our number one priority.
  • develop systems and processes which embed evidence based best practice
The successful candidate will work as part of a team which will provide the service group with assurance that all serious incidents have received appropriate scrutiny and that learning has been identified and actioned.
