The Community Response Service team provide care to patients in their own home who are at risk of hospital admission and we help promote an early discharge for patients who would otherwise remain in hospital.

The Community Response Service provides intensive treatment, care and monitoring to patients in their own home. The team complement and utilise existing community therapy and nursing teams.

The successful candidate will lead on the care of patient and be responsible and accountable for the physical delivery of therapy intervention in the patient's usual place of residence and to a defined caseload. They will utilise team management and organisational skills when supporting and supervising junior members of staff

The successful candidate will have both clinical and operational responsibilities however this may fluctuate depending on service demand however we foresee the role being approx. 30% operational and 70% clinical care.

  • To work clinically within the Discharge and Reablement Services holding a caseload of complex clients.
  • To assess for and implement specialist physiotherapy treatments and programmes of intervention.
  • To advise and guide the physiotherapists who work within the designated area to ensure high quality service delivery in relation to the service guidelines, as well as organisational and national guidelines.
  • To monitor the performance of the physiotherapists working in designated area and offer feedback to support clinical improvements.
  • To work a as resource for all physiotherapists working in designated area to ensure the maintenance of equitable service delivery.
  • To participate in the supervision of all physiotherapy staff within designated area in the form of peer review, live supervision, clinical meetings and formal one to one’s.
  • To participate in annual staff appraisal.
  • To ensure the progression of Continued Professional Development and the retention of documented evidence in the form of portfolios.
  • To role model compassionate and inclusive leadership in order to shape the creation of a collective leadership culture within the trust. This means demonstrating a consistent leadership style which (a) engages, enables and empowers others (b) uses coaching to promote ownership of learning and quality improvement and (c) facilitates team working and collaboration within teams / departments and across organisational boundaries
We manage three major locality hospitals at North Tyneside, Wansbeck and Hexham, plus a number of smaller community hospitals and clinics from Tynemouth to Berwick on Tweed, in addition to our state-of-the-art Northumbria Specialist Emergency Care Hospital, the first of its kind in England. We also care for people in their homes and provide services from facilities in local communities such as health centres. We give people greater choice and control over their care to help them to live independently at home and avoid hospital admission where appropriate.   • To liaise with other physiotherapists to create integrated pathways of care and to ensure best practice on behalf of the designated client group.
  • To maintain a high level of personal / professional competency and to appraise that of others.
  • To demonstrate the ability to self-appraise/monitor.
  • To demonstrate the ability to perform as a reflective practitioner.
  • Makes clinical judgements on complex conditions.
  • Evaluate the outcome of physiotherapy and adjust individual. treatment programmes accordingly.
  • Makes judgements on a variety of physiotherapy issues which require investigating analysing and assessing in line with service and organisational requirements.
  • Specialist knowledge and advanced clinical reasoning to support the assessment and diagnosis of a complex caseload and compare a range of treatment options using advanced clinical reasoning skills.
  • Plan, organise and prioritise own complex clinical caseload within team.
  • Plans and maintains a complex caseload.
  • Plans and maintains relevant communication to other physiotherapists within designated area
  • Support the day to day organisation of the speciality team, delegating to qualified and unqualified staff supporting them in the management of their caseload.
  • Responsible for the organisation of peer group meetings, governance meetings and in-service training.
