You would work within our established multi-professional pharmacy team and be supported to develop in order to provide high-quality specialist clinical pharmacy services to our community teams and ensure patients receive the safest, most appropriate, individualised treatment.

To provide and support the delivery of a high quality, clinically focused medicines optimisation service across Lincolnshire Partnership Foundation Trust.

Actively participate in the clinical technician team service in agreed designated areas.

Ensure that a detailed patient medication history and medicines reconciliation is taken at the time of admission.

To ensure that each service users medication summary is prepared for their discharge, and then counsel patients in readiness for discharge on their medication.

Ensure any discharge medication is passed on as part of the Discharge Medicines Service to the patient's GP, and regular community pharmacy.

Liaise with primary care staff on medication issues relating to admission and discharge of service users and supports their engagement with primary care.

To ensure the service provided is patient-centred, safe and supports the cost-effective use of medicines and resources and to participate in developing innovative ways of working to enhance patient-care.

To develop and participate in basic training and teaching activities relating to medicines management across the Trust to other healthcare professionals, service users and carers where required.

To participate in and help develop medicines management audits as required and help develop and implement plans to promote quality improvements in practice.

Clinical and operational:
a. To provide pharmaceutical support to inpatient wards, CMHTs and other integrated services, including crisis and home treatment teams, place based teams as appropriate. This includes providing a named patient medication supply and stock top up service in line with departmental standard operating procedures.

b. To check each patient’s medicines to ensure all medicines held match requirements of the prescription, remove any medicines that have been stopped or changed for disposal.

c. To review prescription charts on site visits according to SOPs, ensuring that the supply of medicines is provided and maintained and that supply information is recorded in a timely, accurate manner to avoid delays in treatment or missed doses.

d. To ensure staff are well informed of supply problems from the provider and any possible treatment delays are planned for to avoid quality of patient-care being compromised.

e. Check re-written charts to ensure that information transferred is complete and accurate. Highlight any immediate problems with prescriptions to Nursing/Medical staff and communicate to the designated Pharmacist.

f. To be able to work independently (not under direct supervision) and travel to various sites within the service to deliver medicines optimisation services.

g. To facilitate patient’s transition through care settings, including continuation of medication supply and effective transfer of discharge information to outpatient teams and primary care.

h. To obtain and verify an accurate drug history for each patient on admission to hospital or within the community setting and carry out medicines reconciliation level 1 intervention. Any discrepancies should be evaluated and discussed with the relevant prescriber and referred to the clinical pharmacist for further advice or clarification.

i. To assess patient’s own drugs (PODs) for suitability of use and ensure that all appropriate documentation is complete e.g. consent forms to use PODs, removal and disposal of PODs.

j. To carry out assessments for self administration of medicines with patients in conjunction with the rest of the multidisciplinary team, order supplies of medicines for self administration and counsel patients on the appropriate use of their medicines.

k. If appropriate, carry out compliance aid assessments for patients and arrange for appropriate compliance aids to be ordered when necessary.

l. To record all significant clinical interventions made during visits. Discuss interventions with prescribers and the multi-disciplinary team as appropriate and follow up outcomes with relevant clinical staff.

m. To monitor the supply of medicines service provided by the dispensing providers and to communicate any issues to the Pharmacy Operational Services Manager.

n. To effectively communicate and collaborate with external providers and anticipate problems within the supply chain to ensure safe delivery of medicines.

o. To assist in the implementation of the Lincolnshire Formulary by highlighting non formulary items and referring to the relevant pharmacist.

p. To ensure regular stock reviews are carried out with the ward/team managers and relevant pharmacists and implement any amendments accordingly.

q. To cover the role and duties of the clozapine lead technician’s role during sickness or annual leave absence, if appropriately trained.

r. To support the monitoring and review of prescribed medication, including accessing relevant blood reports, side effect assessments and care plans.

s. To competently counsel patients on medication queries, supplying information and answering questions, within own competency, once appropriately trained or advised by the supervising pharmacy team member.

Governance and risk management:
a. To ensure all medication incidents are reported in accordance with Trust policy.

b. To assist the ward or lead pharmacists in investigating errors in prescribing, dispensing, drug administration or security issues.

c. To participate in and help develop medicines management audits as required and help develop and implement plans to promote quality improvements in practice. Including, but not exclusive to monthly CD ward audits and annual safe and secure handling of medicines audits.

d. To support the local implementation of the Trust’s medicines policies, guidelines (including NICE) and procedures within own wards/teams.

Training & Education:
a. To be available to carry out basic level service user advice and counselling service using tact, persuasion, empathy, sympathy and sensitivity to answer questions and help overcome any barriers to compliance. This may mean encountering aggression, physical or verbal abuse, language or learning difficulties.

b. To participate in medicines related training to pre-and post-qualification clinical staff.

c. To assist other staff in the development or review of educational material.

d. To support and implement antimicrobial stewardship initiatives across the trust.

e. To support students as required.

f. To participate in information events including the preparation and delivery of presentations to support the work of the pharmacy service.

g. To participate in clozapine blood analysis machine training (POCHI) and be competent in running a clinic without supervision, if appropriately trained.

h. Providing specialist expertise, where competent, to address both the public health and social care needs of patients, including lifestyle advice, service information, and help in tackling local health inequalities.

i. To ensure ones own statutory and mandatory training is completed and maintained.

Financial:
a. To assist in identifying savings that could contribute to the Trust CIPs/locality savings, and/or to the QIPP agenda that may impact across the healthcare community.

Service development:
a. To support the local implementation of the General Pharmaceutical Council’s guidelines and the Trust’s medicines policies and procedural documents within clinical areas including the administration and control of medicines trust-wide.

b. To provide information and advice on medicines management aspects of clinical governance in line with the trust’s policies and procedures.

c. To assist the clinical pharmacists in writing locality/Trust wide procedural documents as required. These procedural documents would be related to activity that impacts on other disciplines.

d. To assist in service development and help shape service re-design within own area. To suggest changes that may affect work or processes involving other professions.

e. To participate in and support audits, devise action plans and support the implementation of those action plans.

f. Be able to access and use all relevant/available patient record systems and have the knowledge and skills to interpret them and/or refer to a pharmacist where necessary.

g. To support and develop systems to improve the quality of discharge medication information available to primary care and help improve the utilisation of Choice and Medication resources.

h. To work independently undertaking medicines management duties in accordance with relevant SOPs and policies, liaising with the appropriate pharmacist as necessary to ensure a seamless service for patients and appropriate use of pharmacist and technician skills.

i. There may be on occasion, a requirement to carry out additional tasks; this will be dependent upon factors such as workload, staffing levels and the strategic objectives of the network.

Other:
a. To participate in education/training programmes to develop skills as part of a commitment to continuous education and the concept of lifelong learning. This will include attendance at appropriate training courses, conferences and meetings of special interest groups.

b. To maintain responsibility for their own professional revalidation and for ensuring that their competencies for all aspects of the job role remain current.

c. To keep up to date with and adhere to current and best practice.

d. To maintain sensitive and confidential information at all times in accordance with General Data Protection Regulations and Caldicott guidance and Trust Information Governance procedures.

e. To be flexible in travelling between sites within the Trust and for external training as required.

f. To undertake any new and changed responsibilities that are appropriate to the job and grade in order to support the evolving work of the team.
