Job description

An exciting opportunity has arisen for a Band 4 Senior Nursery Nurse to work within the Neonatal Community Outreach Service, hosted by UHCW. The post holder will assist neonatal staff across the Local Maternity and Neonatal System (LMNS) at 3 hospital sites (University Hospitals Coventry (UHCW), George Eliot Hospital (GEH), and South Warwick Hospital (SWFT)) with early and appropriate discharge planning in accordance with the criteria for neonatal outreach care.

The service provides continuing community care for babies and their families in the home setting. The Neonatal Community Outreach Service provides ongoing care and support for babies discharged home requiring nasogastric tube feeding, oxygen and home phototherapy.

We are seeking a highly motivated Senior Nursery Nurse to join our forward thinking family focused neonatal team. The successful candidate will demonstrate the drive and motivation to support the implementation of such an exciting service across the sites. To be successful in this role you will need to have had clinical experience of working in neonatal care, and to have successfully completed the Fundamentals of Neonatal Care course.

The post holder will be an excellent team worker and will be responsible for ensuring the highest standard of clinical care and evidence based practice for a defined group of infants within the scope of neonatal outreach.

Hours of work will be a mix of 08.00-16.00 and 10.00-18.00, Monday to Sunday.

To Support the delivery of the Neonatal Community Outreach Service and
ensure a high quality, efficient and effective service, in support of an integrated LMNS approach.

To work within the hub and spoke model for the outreach team.

To provide feeding advice and support for families cared for in the community.

To support a seamless timely discharge from hospital to the home environment.

Collect data as required to support the outreach model
Support patient pathways and facilitate early discharge of patients.

Demonstrate technical and/or practical skills to ensure optimum patient management e.g. patient observations, equipment management and administration of agreed invasive tests/procedures, and defined expanded clinical roles.

Responsible for own actions, working within agreed guidelines and polices, including all aspects of child protection; ensuring the safety of the baby at all times, recognising the need to refer to a trained member of staff as appropriate.
