Reablement is a free service that is provided post hospital discharge at the clients home. It is a short and intensive service, usually up to six weeks.
This service is offered to people with disabilities, who are frail or who are recovering from an illness or injury.
The purpose of reablement is to support these people in relearning skills and abilities and regaining confidence that are required to keep them safe and independent at home.
The aim is, that over six weeks the individual will be supported to regain physical functioning, relearn skills and if necessary learn different ways of doing daily tasks such as meal preparation, washing or dressing.
First there needs to be an assessment of what the person is able to do and what only limited or not at all.
As it is a quite intensive service, it is likely that reablement workers visit more frequently and stay for longer than in normal home care.
Reablement worker will actively assist people in doing things for themselves and not do the things for them. This means they are more likely to stand back and give encouragement, or show the person new ways of doing things. This usually takes longer than in normal home care. Motivation and encouragement is very important in reablement. Remind the people what it means when they are able again to do things for themselves.
During reablement, workers will measure how the person is improving as time goes on to record progress and plan their ongoing support.
