This is an exciting opportunity to join The Swansea Bay COPD team.
The team care for patients with COPD to prevent avoidable admissions to hospital and provide an early supported discharge when safe to do so.
The team work with patients to provide education and support them to have the best quality of life.
Patient satisfaction is extremely high which leads to job satisfaction for the team .The majority of care is provided in patients homes across the Swansea Bay area.
The successful candidate will be supported to develop their skills to be able to provide a very high standard of care within a team of Band 6 and 7 Clinical Nurse Specialists and Physiotherapists.
Providing education and teaching self management skills
Recording observations
Spirometry ,phlebotomy, ecgs, inhaler technique
Monitoring weight and initial nutritional support
Recording patient reported measures of disease burden and satisfaction surveys to ensure the service is meeting patients needs
