Senior Healthcare assistants will provide an essential link between our Nurses and Healthcare assistants in the clinical care of our residents.

They will directly report to the Registered Nurse in charge of the shift and are responsible for coordinating the healthcare assistance, acting as a role model in the delivery and promotion of best practice and person centred care at all times. To support our residents in areas including but not exclusive to their nutritional requirements, social activities, mobility and wellbeing.

Role purpose:

Offering continuity of care and ensuring residents physical, social, cultural and spiritual needs are met. Overseeing a team of Healthcare assistants under the direct supervision of the Registered Nurses.

Work within the Code of Conduct for Healthcare Support Workers and Adult Social Care Workers in England and the Care Quality Commission Essential standards of Quality and Safety.

2. Promote compliance with Health and Safety, COSHH, Infection Control and Food Hygiene at all times.

3. Assist the registered nurses with clinical care of residents.

4. Deliver individualised, cultural, spiritual, social and end of life care to residents.

5. Under the guidance of the registered nurse, contribute to the operation of the units in a cost effective and efficient manner.

6. Under the direct supervision of the registered nurse triage the work load to healthcare assistants. Promote and act as a role model for best practice and patient centred care at all times.

7. Build relationships with family, friends and significant others. Involve them in activities and care as appropriate and ensuring they remain a large part of the residents day to day lives.

8. Contribute to the completion of daily documentation for example, food & fluid charts, daily records and observational charts.

9. Act in accordance with Lexicon policies and procedures at all times.

Main Tasks

Nursing and care:-

1. Deliver person centred care including personal care, oral hygiene and appliance application (hearing aids, splints etc.)

2. Application of Topical creams and charting on the TMAR chart.

3. Delivering appropriate end of life care with in Gold framework standards.

4. Undertake weights, Baseline observations and continence assessments. Ensuring they are charted in the appropriate section of CARSEYS and report concerns to the registered nurse in charge.

5. Observe, record and respond to changes in resident’s mental health, health or wellbeing.

6. Co-ordinate and ensue the effective use of the key worker role amongst the Health care assistants.

7. Act as a liaison between family, friends and other health care professionals and the registered nurse in charge.

8. Assist and co-ordinate meal times with the lead host. Ensure those resident needing assistance are supported and that the residents are able to make informed nutritious choices from the menu. Ensuring all Healthcare assistants follow SALT & Dietary guidelines (Food texture descriptors)

9. Ensure the safe removal of clinical waste from the sluice areas to the receptacle in the bin store.

10.General care and cleaning of equipment and wheelchairs – reporting to nurse in charge or maintenance manager any issues arising in the appropriate manner.

11.Escort residents to appointments, activities. Ensuring appropriate level of care remains in place whilst other staff off site.

12.Ensure rooms are stocked but not excessively, receiving and putting away stores as needed. To ensure the stock request form is completed in the stock room if stores are low.

13.Play a key role in the greeting and settling of new residents and there initial assessments.

14.At all times promote and ensure a high standard of care and the reputation of the home.

Additional tasks:-

15. Once assessed as competent by the Heads of care; under the guidance of the registered nurse to undertake clinical tasks which may include:

· Blood sugar monitoring

· Simple repeat dressings

· PEG administration and flushes

· Observations

· Catheter care

· Colostomy care

· Limited Medication management

· Venepuncture

16. Use Dementia friends training and act as an ambassador for this role and support Healthcare assistants to engage and support our residents living with Dementia.

17. Deal diplomatically and sensitively with residents who have difficult, inappropriate or challenging behaviour. Step in to relieve other team members as necessary; but also support and empower them to try and deal with the situations themselves.

18. Support the OT & Head of Care to ensure prescribed manual handling plans are carried out and best practice is followed at all times.

19. Under take supervisions of the Healthcare assistance under direction of the Heads of Care.

Managing performance and service delivery:-

20. Act as a role model to Healthcare assistance, work experience students and Volunteers, promoting best practice, person centred care and up keeping the homes philosophy of clinical and caring excellence and in compliance with the homes policies and procedures.

21. Under the direction of the registered nurse, be responsible for the coordination and smooth running of the unit assigned to that shift. Triaging the work load in line with the allocation sheet for the day. Be aware of the balance between numbers and skill mix.

When used oversee bank & agency workers on shift.

22. Ensure staff levels are maintained. Encourage and promote a flexible culture with in the team. Identify staff shortage solutions for the senior team and be aware of shift rota and general resource management.

23. Deliver informal coaching and guidance when identified and appropriate when spotted ‘on the ground’ (poor manual handling technique etc.) Identify training needs and refer them to the Head of Care or Home Director.

24. Trouble shoot issues or problems with residents and/or team members, referring concerns to the appropriate level when necessary.

25. Support nurses in the induction of new healthcare assistants. Monitor progress and act promptly on key development needs. Introduce new staff to key personnel and colleagues, familiarise with the units. Monitor performance at a local level in the first few weeks and act as a mentor for new joiners. Support completion of new starter paperwork.

26. Motivate and inspire healthcare assistants in the delivery of a high a high quality service in line with our ethos of care and to work effectively and efficiently as a team.

27. Attend and contribute to staff meetings and communications – regularly checking memos, notice boards and in trays for general information. Ensure information is filter down to the team.

28. Undertake after training and assessment the healthcare assistant’s supervisions in line with current policy.

29. Maintain own skill set and knowledge on best practice, care and dementia care legislations/development. Attend mandatory updates.

Monitoring and Record keeping:-

30. Under the direction of the registered nurse ensure all documentation both paper and electronic is completed and maintained for inspection at all times. Paperwork should be done on a ‘as it happened’ scenario instead of at the end of each shift. This is not effective or accurate charting.

31. Maintain team communication records – embed information-sharing practices as part of the team working ethos of the home.

32. Work in compliance with best practise in, fire safety, health & safety regulations and CQC Essential standards of quality and safety. Help maintain a safe environment for residents and staff. Report defects, faults, incidents and accidents in accordance with the homes policies and procedures. Ensure attendance to mandatory updates.

33. Comply with the data protection act 1998 (the general data protection regulation (GDPR) will come into effect on 25 May 2018) and ensue residents or staff information is not compromised at any time. Keep passwords safe and change regularly. Ensure door to nursing station/reception is kept shut when not in use.
