We are looking for an enthusiastic, highly motivated person who has an inclusive approach to special educational needs and who can help the children and young people of Lancashire to achieve their full potential in education, learning and future employment. You will be able to support the council to fulfil its statutory duties in relation to children and young people with special educational needs.
The caseworkers support the compliance with the legislation and government guidance in meeting the educational, health and social care needs of children and young people with special educational needs and disabilities.
They support the reviews for children, young people and families are carried out within statutory timescales with decisions which result in outcomes that meet the identified needs they work closely with other teams within the Service and with other services in the Directorate to ensure assessment processes are coherent, comprehensive, and transparent and involve children, young people and their parents/carers.
The successful candidate will also:
• Provide a case management role to children, young people (CYP) and their families who are undergoing an education, health and care assessment or have an education, health and care plan, across the 0 - 25 age range, including the management of complex cases.
• Provide case management functions that enable effective and efficient support in order to build strong and resilient children, young people and families.
• Co-ordinate a person centred, outcome focused multi agency approach to assessment, planning and support ensuring that parents are treated as equal partners in the assessment and planning process.
• Liaise with schools, young people, families and professionals in order to support the maintenance of the EHCP.
