About the position
The Care Coordinator is responsible for receiving, processing, and coordinating all new patient leads for Anova Fertility and Reproductive Health and coordinates the care of patients. Works closely and collaboratively with operations and the third party program to initiate an appropriate and timely start of care for the new patient.
Responsibilities

· Exercises confidentiality and protects the privacy for the greater good of the company
· Adheres to Anova Fertility and Reproductive Health’s professional behavior standards
· Adheres to established attendance schedule
· Performs other departmental, interdepartmental, and organizational duties/tasks as needed and requested
· Receiving referrals from referral sources and coordinate services requested for the patient
· Arrange provider of service based on patient clinical need and geographic location
· Troubleshoot and resolve problems/concerns relative to services with patients referrals
· Maintain open lines of communication with all potential patients and physicians offices to facilitate optimal experience and care
· Advocate for the patient regarding best use/most appropriate service relative to their needs
· Troubleshoot and assist in the resolution of any start of care issues/concerns
· Participate in collection and reporting of quality issues/concerns to senior management
· Participate in the on-call schedule
· Assist in orientation of new staff as required
· Protect all medical information under HIPAA privacy for PHI and HIPAA security for EPHI
· Adhere to Corporate Compliance Plan related to fraud, waste, and abuse Adhere to Code of Ethics and Business Conduct Plan
· Abide by organizational policies and procedures regarding business and medical data
· Give highest priority to creating exceptional customer experience
· Anticipate and honour customer requests, needs and issues with timely follow through
· Handle and address customer complaints/concerns in empathic pleasant manner
· Ability to remain calm under pressure and resourceful in challenging and tension-filled situations
· Exude results-oriented, friendly, and positive-minded attitude and manner to internal staff and external customers
Qualifications
· Strong interpersonal skills
· Highly resourceful team-player, with the ability to also be extremely effective independently
· Knowledgeable of standard Microsoft Office programs
· 3 years’ experience in care coordination and intake or equivalent sales experience
· Communicating (phone, fax, email), collaborating, and planning with operations/physician office, initiation of patient’s care program
· Set high standards and goals for self, role, and performance
· Demonstrate pride in work team, and organization as a whole
· Complete work on schedule; sets and meets expected timelines
· Remain aware of personal and team needs, short term and long-term objectives

Salary is $80,000-$100,000 OTE
