Pennine Care Foundation Trust is looking for2 specialist practitionersto deliver a range of trauma informed, mental health and dual diagnosis services for individuals experiencing or at risk of homelessness acrossOldhamandRochdale. These posts (one per borough) will work alongside a range of public and third sector providers to support the needs of this population. This can include those who are street homeless, in transient accommodation (such as sofa-surfing) and those who are at risk of becoming homeless due to the nature of their current accommodation provision. Our services work in a trauma informed, psychological model, considering the wholistic needs of the individual including health and social care needs.
Role Summary:
The post holders will be highly skilled and competent in conducting comprehensive holistic health and social needs assessments and risk assessments, developing outcome based needs led care plans, facilitating the implementation of the agreed plan of care and evaluating its effectiveness utilising evidence based outcome measures with a focus on recovery and social inclusion.
The post holders will also act as a specialist resource for other staff across the service who will be working with this client group.
