GMMH is delighted to be able to offer 3 care coordinator positions within a new and exciting Rough Sleeper CMHT, based both within the Homeless Hub and the Manchester CMHT's.
This is a new and dynamic team that will offer care coordination for homeless service users, working to an assertive outreach, trauma informed approach. The team will link in with the wider homeless voluntary sector and GMMH services in Manchester.
The post holders will be supported by a specialist multi-disciplinary, trauma informed Rough Sleeper CMHT and will work with individuals who are experiencing mental health problems and are rough sleeping. Caseload capacity will reflect the need to offer an assertive outreach approach which is engaging, consistent and flexible for clients.

The post holders will provide care coordination to clients with complex mental health and substance misuse (dual diagnosis) needs who are rough sleeping within the city of Manchester.
The post holder will be part of the GMMH Homeless Hub and Manchester CMHT's and will work with voluntary agencies when appropriate providing mental health support to rough sleepers.
The post holder will be responsible for working within the CPA framework. The role will include active participation in the service planning and future development of the mental health support for homeless individuals within the Manchester district.
