Torbay and South Devon NHS Foundation Trust is an integrated care organisation which aims to provide high quality, safe and effective health and social care and we have an exciting opportunity available within our Intermediate Care and Community Therapy Team.
We are looking for a Band 6 Physiotherapist with experience or a Band 5 Physiotherapist looking for career progression to join our rotation. The rotation areas are in the Moor to Sea Intermediate care team and the Community Hospital ward at Totnes Hospital.
As required you will also support the Community Physiotherapy team, including running Strength and Balance Groups
The Community Hospital ward is a 18 bed unit and the Therapy team provides assessment and rehabilitation to people with a range of medical conditions. The post holder will manage the caseload and delegate to support workers as appropriate. They will take an active role in service development.
As part of the Intermediate Care team the post holder will work as part of the multi-disciplinary team contributing to joint decisions regarding the management of patients, including acutely ill patients, referred by their GP or paramedic. They will triage and prioritise work according to urgency and presenting risk. They will undertake advanced physiotherapeutic assessment of patients with diverse conditions and provide physiotherapy treatment programmes.
The post holder will be well supported by senior staff as well as the Community Therapy Team and Intermediate Care team. There will be an opportunity to supervise support workers and students. There are excellent CDP training opportunities, including Physiotherapy peer support groups including Clinical Interest Groups and training from the South Devon Health Services training Team.

The Community Therapy Team provides Therapy input to the Community Hospital ward at Totnes and also provides Physiotherapy and Occupational Therapy in the community, within the Moot to Sea locality.
The Intermediate Care Service facilitates crisis management in the community, prevention of hospital admissions, quicker discharge from hospital and supports people to remain as independent as possible at home. Patients are seen within the most appropriate setting; at home and in Intermediate Care placements in residential and nursing care homes. The team works in a truly integrated way, with nurses, social workers, occupational therapists, pharmacist, dietician and paramedics.
