Shift
Primarily days, Monday – Friday (40 hours per week). Some evenings and weekends may be required as this is an exempt, hourly position.
Job Summary
The Institute for Biomedical Informatics (IBI) has contracted with the Cabinet for Health and Family Services, Department for Medicaid Services (DMS) to create a team of licensed Registered Nurses that will perform assessments utilizing the Home and Community Based (HCB) waiver assessment tool for the frail and elderly population and physically disabled. This position will identify, develop and present information educating professional staff, families and individuals seeking HCB Services while also making sound independent determinations as to the level of care and services needed for this vulnerable population.
This position is located in Jefferson County and entirely remote. You will work out of your home and travel within your assigned county to conduct in-home assessments. You will also have flexibility when scheduling in-home assessments to meet client needs. (Travel outside of the county may be required on an as-needed basis.)

Skills / Knowledge / Abilities
Knowledge of physical and psychological development. Excellent analytical and decision making skills. Ability to communicate effectively and interact with physically disabled and elderly individuals. Ability to travel throughout the state on a regular basis with the ability to travel out-of-state for overnight trips upon occasion.

Preferred Education/Experience
Prefer experience working with the elderly and physically disabled. Driver’s license is highly desired.
