An exciting opportunity has arisen for a Band 3 Rehabilitation Care Workers to join Barnet Unplanned Care Service (Urgent Community Response). We have a number of pathways within the service our main aim is to prevent admission to hospital and support early discharge from the acute. Pathways within the service are, D2A, NWB, Delirium, Enhanced Nursing and Virtual ward.
The Unplanned care team provides short term support to patients.
The Rehabilitation Support Worker’s role will be to support the nursing team. delivery of equipment, taking specimens to the local hospital, taking observations, and with training phlebotomy.
The successful postholders will perform nursing and therapy procedures where necessary for which adequate training has been given.
They will work under the supervision of the professional staff in the Unplanned care team (UCR) in the rehabilitation and nursing of patients.
To work to agreed standards of care within the Team and always to consider the needs and wishes of the patient and their families.
To be highly organised with excellent verbal and written communication skills and experience of being able to follow Therapy and Nursing care plans and treatment plans in a community environment.
Possesion of UK driving license and access to a vehicle for business purposes is preferable . As you will be required to deliver equipment such as commodes and trolley, pressure releiving equipment and dropping urgent bloods to the hospital.
Successful postholders must have an ability to work 37.5 hours across 7 days per week. Hours of work are 8am - 10pm on two shift pattern 8am - 4pm and 2pm - 10pm.

- To be responsible for a caseload of patients working to agreed treatment objectives and to assist qualified staff in carrying out individual treatment programs
- To provide therapy and nursing interventions as part of a multidisciplinary team to patient within an agreed scope of practice
- Successful completion of core competencies assessed by qualified staff
- To be responsible for handing over patient progress and any areas of concern following intervention to the relevant therapist or Nurse
- The Rehabilitation Support Worker’s role will focus on improving independence and will include promotion of daily living skills, personal and social care.
