Rehab Support Worker iCares West Bromwich

Sandwell and West Birmingham NHS Trust
Full Time West Bromwich 10.56 - 12.04 GBP Today
Job description

An exciting opportunity has arisen for us to be able to recruit more Rehab Support Workers to join and enhance our community rehab team. You will join the Integrated Care Service (iCares) which works across Sandwell providing rehabilitation, case management and admission avoidance to people in their own homes. These are often people recovering from an illness, a hospital stay or increasing frailty. With our support and specialist rehabilitation we aim to improve their physical function such as walking, looking after themselves and even returning to community activities again. Within the Team we have Occupational Therapists, Physiotherapists, Community Matrons, Speech and Language Therapists, District Nurses, and Assistant Practitioners all working together to deliver integrated care. Please note we reserve the right to close this vacancy early if a suitable number of applicants apply. The Trust aims to be the most integrated organisation in the NHS with support for new ways of working and innovation. As a result of this Sandwell Community is an exciting place to be with new investment and ever closer working with colleagues from the local authority and primary care. Everything we do in the iCares Directorate strives to add value to the patient and their community. We do this by joining up our teams and services to keep people safe and well in their community. We have fantastic links with partners across the local authority and third sectors in order to do this. We are a friendly team who support each other. Training and regular supervision are available and no one works in isolation. As a trust we fully support flexible working and part time workers, which can be factored into your working pattern. Within iCares we operate a 7 day service 8am-8pm, we take it in turns to contribute to weekend working and 8pm finishes. Work is only one part of life. We are committed to helping colleagues achieve the best possible work/life balance. We are proud to offer supportive, inclusive and family friendly employment and flexible working policies. We have a wide range of professional services and employee networks to help our colleagues be at their best and find support if they need it. Candidate must hold a valid UK driving licence and have a vehicle which they can use for work purposes. Mileage can be claimed for journeys at the usual NHS rate. Sandwell and West Birmingham NHS Trust (SWBT) is an integrated care organisation with a budget of approximately £600 million and over 7,000 staff. Diversity and social care is at the core of what we do as the Trust provides Community and Acute Services to over half a million people in an urban centre that demands massive regeneration and has substantial premature mortality. Our new acute hospital, the Midland Metropolitan University Hospital (MMUH), opens in 2024 and will provide care to our local population from first class, purpose-built premises. As a result, the base of this role may change to MMUH from 2024 (or beyond). If this is applicable to your role, you will be informed during the recruitment process and continuing your application with this understanding. The development of the new hospital will play an important role in the regeneration of the wider area and in improving the lives of local people and reducing health inequalities. We have three newly emerging strategic objectives: 1. Our People – to cultivate and sustain happy, productive and engaged staff 2. Our Patients – to be good or outstanding in everything we do 3. Our Population – to work seamlessly with our partners to improve live Please see the attached Job Description and Person Specification for full details of what this post entails.
