This position may be eligible for the Education Debt Reduction Program (EDRP), a student load payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. This position may be eligible for recruitment, retention, and relocation package.
The position will require provider to be on site at Ashtabula Community Outpatient Based Clinic, 2044 Lambros Lane, Ashtabula, OH 44004, 2-3 days per week and remaining time would be via telework. Days on site are negotiable. It is a Monday through Friday from 8:00am to 4:30pm.
Looking for a dynamic excellent full-time certified Psychiatric Nurse Practitioner that demonstrates leadership in delivering and improving holistic care through collaborative strategies functioning as the advocate for patients and is clinically supportive of staff. The incumbent will be responsible and accountable for all elements of the advance nursing process when providing and /or supervising direct patient care. The nurse will be experienced in the coordination of care focused on patient transitions through the continuum of care, patient and family education, patient self -management after discharge, and supporting factors that impact customer satisfaction for the patients in the Behavioral Health. Communication skills are essential to optimize collaboration with Behavioral Health Interdisciplinary Program (BHIP) and providing team-based care.
Psychiatric-mental health advance nursing practice is characterized by interventions that promote and foster health, assess dysfunction, assist patients to regain or improve coping abilities, and prevent further disability. These include diagnosis, conducting therapy, intake screening and evaluation, providing emergency psychiatric services including recognition of differential diagnosis of medical disorders with psychiatric symptoms or disorders. This position includes use of evidence-based practice, case management; provision of a therapeutic environment, prescribing and monitoring psychobiological treatment regimens to include psychobiological treatment regimens to include prescribed psychopharmacological agents and their effects; health teaching; crisis intervention and counseling utilizing a Whole Health System of care & trauma informed care. This is an approach to healthcare that empowers and equips people to take charge of their health and well-being and live their lives to the fullest. Provides direct nursing care in a self-directed manner.
Core elements of performance include knowledge and active participation in program level quality improvement processes and initiatives as well as customer service programs. Actively supports and actively participates in nursing facility wide initiative, trainings, and/or committees.

Full time Registered Nurse position available at the Erie VA Medical Center. Duties will be primarily performed within the Community Based Outpatient Clinics (CBOCs), however, occasional reassignment may be required based on patient care needs
Duties
Responsible and accountable for all elements of the advance practice nursing process when providing and/or supervising direct patient care.
Adheres to established policies, standards of care and standards of practice. Provides direct nursing care in a self-directed manner.
Assesses, plans, implements and evaluates care based on age-specific components.
Assumes responsibility for the coordination of care focused on patient transition through the continuum of care, medication management, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction.
Diagnoses, conducts therapy, prescribes psychotropic medication for patient who have psychiatric, medical mental conditions, or substance abuse problems. Comprehensive knowledge of the Diagnostic and Statistical Manual of Mental Disorders (DMS) and applies criteria to make an accurate diagnosis.
Monitors medication adherence and effectiveness an active member of the Behavioral Health Interdisciplinary Program (BHIP) and provides care in a team-based appr Influences care outcomes by collaborating with members of the interdisciplinary team to coordinate and develop the Veteran's treatment plan, incorporating the Veteran's/family's input and considering all characteristics of the individual, including age and life stages, state of health, race and culture, values, and previous experiences.
Demonstrates a systematic body of knowledge to the diagnosis and treatment of actual or potential health problems, including syntheses of data, and the use of complex skills and interventions. Monitors Veteran's treatment plan and documents progress in care delivered, outcomes achieved, and goals attained. Communicates with the Veteran about the treatment plan, including long range mental health care and addresses any problems or concerns about their care.
Considers all characteristics of the individual, including age and life stages, state of health, race and culture, values, and previous experiences.
Exercises independent judgment with prescriptive authority as noted in their written practice of agreement as agreed upon with their collaborating psychiatrist. \

Qualifications

Physical Standards: All applicants must meet the physical standards for the position. A physical examination prior to employment may be required.
Licensure: Full, active, current, and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia.
Education: Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC)] at the time the program was completed by the applicant. Health care education in the military service or training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure will be accepted as meeting the education requirements for VHA employment. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; student; social).
