Job description
To support and assist in the management of the community Tissue Viability and Lymphoedema Service for Guildford and Waverley. To provide strategic leadership and management for wound cares services and proactively develop the service to meet the trust values and visions. To proactively support engagement with other partner agencies to share excellent pressure ulcer/leg ulcer prevention and management practice and implement shared skills and knowledge with these key partner agencies. To use expert knowledge to diagnosis lymphoedema and chronic oedema, identify underlying cause, implement treatment, follow-up and review treatments, promote self management regime for patients.
The post holder will provide highly specialised clinical advice and expertise for patients with multiple and complex wounds and/or skin conditions. The post holder will maintain effective and efficient systems for pressure ulcer reporting, as well as the interpretations and analysis of route cause analysis (RCA) findings for the development of division-wide action planning and patient pathway development. The post holder will provide strong leadership that inspires and motivates others to take individual responsibility in pressure ulcer prevention and wound management across the community patient pathway.
