We are looking for an enthusiastic Qualified Nurse with relevant experience to become part of the Peritoneal Dialysis team who care for patients on peritoneal dialysis at home.
You will be a proven team player with post registration renal experience, including at working at Band 5 level or equivalent. You will have previous experience of caring for patients on peritoneal dialysis.
You will bring evidence of multi-disciplinary and multi-agency partnership working, and the ability to both influence and embrace change. Well-developed communication skills are essential.

Main duties of the job
Key objectives would be the management of patients on peritoneal dialysis, initiation and training of new patients, providing ongoing support and care to the patient and their carers / families.
You will be expected to provide high quality care to this complex patient group,support patients in the their homes and in outlying hospitals . Also provide telephone triage advise and consultation.
You will have a major role in educating and supporting both healthcare professionals, patients and significant others with regards to the patient journey with the ultimate aim of enhancing quality of life.
Non-medical prescribing qualification is desirable but not essential as this training opportunity will be provided.
Due to the nature of the post it is expected that you will have a full driving licence and will have regular use of an appropriate vehicle (as determined by the Trust).

Qualifications

- First degree in nursing or equivalent portfolio
- 2 years post registration experience within the speciality
- Evidence of involvement in clinical governance
