Provide information, advice and support to patients enabling them to fully understand their diagnosis and treatment options thus enabling assisting them to participate in decisions relating to their care management Have skills in the care of individuals with specialist palliative care needs Support the management of patients with specialist palliative care needs through the provision of assessments including end of life, preferred priority of care, and advice on pain and symptom control Influence the quality of care for patients at the end of life Work closely with all healthcare professionals including staff in training within LHCH and external to the hospital Be a source of information on specialist palliative care providing support, care and advice for patients, their carers and health professionals Develop knowledge and skills within the field of specialist palliative care CLINICAL Demonstrate skills / knowledge within specialist palliative care utilising these skills in the assessment, diagnosis and treatment of the patient with the support of other team members as required Subsequently plan, implement and evaluate care delivery according to the patients changing health care needs with the support of other team members as required To provide support to staff including the link nurses within competence Develop therapeutic relationships with patients and their carers / relatives, utilising skilled communication and interpersonal skills Involve patients and carers / relatives in the planning and delivery of their care Act as the identified key worker for the patient / family and carer for the on-going advice and support they require Maintain accurate patient records, collect and collate data to provide evidence of service activity Facilitate and work within a multi-disciplinary team to ensure a coordinated approach to meeting the needs of patients and their carers / families Work collaboratively with the multi professional team both within the Trust and Primary Care to ensure a coordinated approach to meeting the holistic needs of the patients and carers Accept referrals, prioritise care, provide feedback and refer to appropriate services Provide assessments for patients with complex palliative care needs utilising skills underpinned by evidence based approaches to care supported by senior team as required Provide excellent communication support to patients, carers and colleagues to enable service delivery and quality Provide emotional, psychological and spiritual support to patients, carers and health professionals throughout the period of illness, in specific complex cases Provide advice and sign posting for bereavement support to known carers, and signpost other individuals as required MANAGERIAL/LEADERSHIP Maintain effective communication and interpersonal relationships with professionals across the health care and social settings Utilise clinical supervision and personal support network to meet both functional and emotional needs Utilise change management skills to promote efficient / effective change / developments within the service Attend appropriate meetings within the Trust Participate in the development of specialist palliative care services Ability to prioritise and meet competing demands from patients, relatives and members of health professional groups and management, with the ability to work flexibly in a rapidly changing environment Effectively manage self to practice within the components of the clinical nurse specialist role Promote patients equality, diversity and human rights Liaise closely with the multi professional team in planning care for the person with specialist palliative care needs and evaluate the effectiveness of care Support and advise junior staff on ethical issues in relation to end of life decisions with the support of other team members as required Contribute to research based practice associated with palliative care Support clinical audit in order to improve effectiveness of patient care Support the development and implementation of policies, procedures and guidelines relevant to own area of work Promote patient and public involvement activities in the specialist area. Use clinical skills to assist in evidence based practice Undertake and evaluate / audit / to promote development of self and the service Support team with audit projects linked to clinical and organisational priorities of the ICN, Merseyside and Cheshire cancer audit network Support the development and implementation of policies, clinical guidelines, standard operating procedures and pathways for specialist palliative care services in line with national, network and local requirements
