Job description
We are delighted to share we have vacancies for band 5 staff nurses to join our expanding Complex Frailty team . We welcome both experienced and newly qualified staff nurses. The role focuses on meeting the needs of the Frail population requiring complex assessment and decision making to provide the right care in the right place within the first 72hrs of the frail patients admission with a focused patient centered approach to admission avoidance and reduction in length of stay.
You will be part of a friendly,supportive and professional integrated ward team comprising of Consultants, nurses, occupational therapists,physiotherapists,pharmacists social and community services.
