- Position Overview: As a Care Manager, you will be providing care coordination for patients who are returning home from nursing homes or rehabilitation facilities. This job entails extensive telehealth communication with patients, therefore, being personable, compassionate, and understanding are important traits. We want to assist our patients with enhancing their health & overall well-being by building lasting connections with them. You will be making phone calls/zoom calls with patients to set up services for them. Must be comfortable speaking on the phone with patients/facilities to coordinate services that assist our patients age in place. You must also be comfortable discussing behavioral health, end of life planning, and whatever else your patient needs. Experience in health care is a must, so you will be familiar with the needs of the patients, and be able to quickly and successfully assist them. This position reports to the Care Manager Supervisor.
General Responsibilities (including, but not limited to):
- Researching & coordinating various services such as physical therapy, medical supplies, transportation, etc., in alignment with patients’ needs.
- Extensively communicate with health providers, patients, and key contacts, to ensure continuity of services.
- Document patient notes in the electronic health record in an accurate, timely, and detailed manner for approximately 50 patients per month.
- Follow-up on phone calls, emails, and correspondence promptly to ensure quality standards of service delivery are met.
- Perform other duties as assigned.
What will I be doing as a Care manager?
- At MedAssist USA we are here to help our patients receive the services, equipment and knowledge to stay at home and age independently. Your average day will begin with a list of patients, you will work through your patients throughout the day to provide assistance in many forms. You will call your patient to touch base, have conversation, gain information on their needs etc. You will then utilize our system, documents, and research their local area to find things like services, doctors, equipment, or senior services in the patient's area. You will then work on your next patient of the day, following the same routine.
What will I set up for patients as a care manager?
- We are able to assist patients in a variety of services! We set up services such as:
- Home Health Care
- Physical Therapy/Occupational Therapy
- Transportation
- Ordering equipment (a few examples)
- Wheelchairs
- Walkers
- Assistive medical equipment
- Toilet commodes
- Hospital Beds
- Locating senior services
- Researching to assist the patient on managing their chronic conditions
- Provide important reminders for prevention
- Research grants and financial assistance.
- These are just a few examples of many ways we can assist our patients!
Requirements and Qualifications:
- Background in Healthcare
- Positive attitude.
- Strong communication skills.
- Well-versed in computer skills

A day in the life of a care manager at MedAssist USA
- You will work on 4-6 patients per day. Each patient will be worked on for about an hour to an hour and a half. Within that time frame you will call the patient and work on their needs such as ordering equipment, setting up services, or researching prevention and management of chronic conditions.
- You will receive a 30 minute paid lunch to take when you see fit during the day.
- Periodically you will participate in mandatory company zoom meetings, 1:1 meetings with your supervisor or Lead Care Manager, and team meetings.
- Generally, this time is accounted for in your weekly hours (2.5 hours/week).
- You will have access to your Lead Care Manager and supervisor for questions, assistance, and support throughout the day.
