JOB DESCRIPTION AND PERSON SPECIFICATION 1. JOB DETAILS Job Title: Patient Flow Sister/Patient Flow Charge Nurse Reports to: Senior Sister/Matron Band: 6 Unit/Department: Centre for Neurosciences Location Leeds General Infirmary AfC Job No: 2060 2. JOB PURPOSE/SUMMARY To have responsibility for facilitating, enabling and supporting interdisciplinary decision making in order to achieve optimal patient flows and utilisation of available resources within Centre for Neurosciences To actively manage patient flows by designing, implementing and maintaining processes that safely expedite patient placement, scheduling and discharge. The jobholder is responsible for supporting the service with timely and appropriate placement of patients requiring admission to the Trusts bed base.

The jobholder is responsible for supporting the multi-disciplinary team in facilitating timely flow of their in-patient pathway and to facilitate the timely discharge of patients. Current services comprise of the management of patient flows for a range of specialities. PRINCIPAL DUTIES & AREAS OF RESPONSIBILITY Clinical Take a lead role in the creation of capacity to meet demand and liaise with Clinicians and Matrons when insufficient demand is created. Participate in creating systems and processes that maintain and improve patient flows.

Develop effective partnerships with clinical and clerical staff and an environment that fosters open communication and participation. Develop effective working arrangements with A&E that allows the timely and appropriate placement of acute admissions. Participate in developing effective partnerships with other Directorates and their Admission/Bed Management Teams that enables timely and appropriate patient placement. Contribute to the development of operational policies/clinical protocols that optimise patient flows on all sites.

Take a lead in facilitating optimum standards of compassionate nursing care for specific patient groups, whose inpatient bed allocation falls outside of the traditionally recognised speciality bed base. Promote evidence-based practice. Work in partnership with the Infection Control Team to monitor wards closed with infection and adjust patient flows accordingly. Take a lead role in facilitating referrals to primary care services for outlying patients.

Contribute to providing a service that maximises the use of community resources and facilitates the timely and appropriate assessment and/or admission of patients, for the GPs in Leeds. Maintain accurate records in accordance with the NMC guidelines for records and Record keeping (2010) and Trust guidelines. Act in accordance with the Nursing and Midwifery Code. Investigate and respond to accidents, untoward incidents and complaints.

Undertake duties on other wards or departments as and when required by service demands or patient needs. Act in such a way that safeguards the health and well-being of children and vulnerable adults at all times. Is familiar with and adheres to the LTHT safeguarding polices. Education and training Promote evidence-based practice within patient admission/discharge processes.

Promote a positive learning environment where lessons are learnt from patient/carer complaints. Facilitate training and education of staff within the team. Participate in annual appraisal Actively participate in the development of own personal development plan. Complete mandatory training within the defined time period.

Leadership To provide focus for the day to day professional leadership of nursing, administration and support staff within the team and the wards. Accountable for providing visible and credible nursing leadership. Act as a positive role model through the provision of support and expert advice to staff. Lead by example through visible leadership to motivate and empower others.

To participate in ensuring that appropriate arrangements are in place to set objectives, monitor performance, agree and review development needs for the staff members. Ensure effective working relationships and communication is maintained between all grades of staff and professional groups. Facilitate the provision of clinical supervision to staff and support staff in arranging mentorship where appropriate. Work with personnel colleagues to ensure appropriate management of the nursing workforce within the team.

Raise concerns with regards risk, danger malpractice or wrong doing by following the LTHT Whistleblowing policy and supporting processes. SPECIAL WORKING CONDITIONS i) PHYSICAL EFFORT: Frequent standing, sitting, walking, communicating. Ability to walk some distance between clinical areas.

Frequently requires accuracy all the time and manual dexterity. Responding to changes in demand and capacity. Not Frequently but on request: Moving & Handling of patients and equipment. ii) MENTAL EFFORT: Frequent mental effort/concentration required in relation to assessing patient placement needs and capacity creation.

Unpredictable workload, responding to changing circumstances (increase/decrease in demand/capacity, prioritisation of workload). Robust verbal and written communication skills required at all times. Working from an open office and frequently answering mobile phone and pager. iii) EMOTIONAL EFFORT: Exposure to distressed patients and relatives/carers.

Exposure to staff working under pressure. Dealing with complex workload. iv) WORKING CONDITIONS: Temperature should be controlled but may experience hot or cold work environment. Violence & Aggression may experience violence and aggression from patients and/or relatives/carers.

Infection Control exposure to wards with patients with infections.
