To lead the development, delivery and evaluation of clinical, educational, strategic and organisational health failure services for Anglesey and Gwynedd. The role involves providing specialist heart failure nursing input and leadership for all nurses and GPs involved in the care of patients with heart failure care needs across the area.
To influence the development of the service to ensure it is patient centred, safe and efficient and in accordance with the appropriate standards.
Lead Heart Failure Nurse Specialist

We are seeking to appoint a highly experienced Nurse with significant experience in Heart Failure to provide expert leadership to our team of Specialist Nurses in our innovative Heart Failure service. Working alongside operational managers, as the lead clinician you will collaborate with system partners to develop patient pathways in diagnostics and management, implement best practice guidelines and ensure that the team delivers high quality care to patients with moderate to severe Heart Failure.
You will work as an expert resource for the Nursing team, providing education and supervision to facilitate the development of advanced heart failure nursing skills and complex clinical decision making. In addition, the successful candidate will be expected to work independently providing expert care to patients with complex heart failure, working closely with other organisations and agencies where necessary.
