To work as a member of the wider Primary Health Care Team in the community division
To assess, plan, implement and evaluate care under the direction of the team leader.
To ensure the provision of effective, efficient, appropriate, high quality patient care over 24 hours, seven days a week.
To deputise for the District Nursing Team Manager/Deputy in their absence.
To act in accordance with the NMC Professional Code of Conduct and other policies.

A registered nurse post has become available within the Caerphilly District Nursing Team based at Ty Bryn Surgery for a staff nurse who wishes to develop their career within the exciting speciality of community nursing.
